Chronic Neglected Scapho-capitate Syndrome: A Rare Case Report with Management.

Introduction: Scapho-capitate syndrome is a rare variety, usually occur during high-energy trauma leads to fracture of scaphoid and capitate with 180 rotation of proximal fragment of capitate.

Case Report: We present a unique case of chronic neglected scapho-capitate syndrome, in which the rotated proximal fragment of capitate along with the early degenerative changes in capitate and lunate.

Result: Exploration of the wrist by dorsal approach, fracture fragment was resorbed and non-amenable for fixation. The scaphoid and triquetrum were excised. The cartilage between the lunate and capitate was denuded and arthrodesis was performed by 2.5 mm headless compression screw. The articular branch of the posterior interosseous nerve (PIN) was excised for pain relief.

Conclusion: Accurate diagnosis in acute injury is essential for functional outcome. In chronic cases, magnetic resonance imaging is necessary to know the status of cartilage for planning of surgery. Limited carpal fusion with neurectomy of articular branch of PIN can give adequate pain relief and improvement in wrist function.

The scapho-capitate or Fenton syndrome is characterized by an associate fracture of the scaphoid and capitate with rotation of 90° or 180° of the capitate's head. We report a case of this syndrome which occurred in a fifteen years old professional motorcyclist who came to our observation following a high-energy trauma that occurred during the track tests. Through a dorsal access the fractures of the capitate and scaphoid were reduced and stabilized with Kirschner wires.

Introduction: Perilunate and lunate dislocations are uncommon high energy injuries and have extensive soft tissue, cartilaginous and bony damage. The most common pattern is trans-scaphoid perilunate fracture dislocation which constitutes approximately 50% of these injuries. Unusual injury pattern like Scaphocapitate syndrome can occur with fracture through scaphoid waist and capitate that rotates the proximal capitate 180° so that its proximal articular surface points distally.

The authors report about chronic Fenton's syndrome. This rare injury of the wrist is characterized by scapho-capitate fracture accompanied by 180° rotation of the head of capitate and associated perilunate dislocation. Two patients suffering from chronic Fenton's syndrome were treated with pyrocarbon capitate resurfacing prosthesis.

[Vascularized pisiform transfer in place of lunatum for Kienböck's disease].

There is a specific vascularization of the pisiform bone depending on the cubito-dorsal artery, branch of the ulnar artery. The vascularized pisiform bone may be transferred in place of the excised lunatum. The aims are to decrease pain when keeping the mobility, to replace a dead bone, to maintain the distance between scaphoid and triquetrum and radius and capitatum, by preventing the carpal collapse.
