Microwatt Volatile Optical Bistability via Nanomechanical Nonlinearity.

Metastable optically controlled devices (optical flip-flops) are needed in data storage, signal processing, and displays. Although nonvolatile memory relying on phase transitions in chalcogenide glasses has been widely used for optical data storage, beyond that, weak optical nonlinearities have hindered the development of low-power bistable devices. This work reports a new type of volatile optical bistability in a hybrid nano-optomechanical device, comprising a pair of anchored nanowires decorated with plasmonic metamolecules. The nonlinearity and bistability reside in the mechanical properties of the acoustically driven nanowires and are transduced to the optical response by reconfiguring the plasmonic metamolecules. The device can be switched between bistable optical states with microwatts of optical power and its volatile memory can be erased by removing the acoustic signal. The demonstration of hybrid nano-optomechanical bistability opens new opportunities to develop low-power optical bistable devices.

Juvenile dermatomyositis with emphasized vasculopathy is rare, but the most severe form of the disease, with a poor prognosis with relapsing and chronic course or, in some cases, lethal outcome. We present a case of a 19-year-old Caucasian female, who developed severe acute juvenile dermatomyositis with emphasized multisystem vasculopathy, including retinal vasculopathy and maculopathy (cotton-wool spots, retinal hemorrhages, macular edema) at the age of 8. Due to no response to standard treatment protocols and rapid worsening of clinical symptoms and laboratory findings, a TNF inhibitor (infliximab) was introduced after the third week of treatment resulting in complete normalisation of muscle enzyme levels and complete resolution of eye changes within the next 2 weeks with a gradual general recovery.

Selective diet induced nutritional optic neuropathy in developmentally normal children.

Purpose: Nutritional deficiencies in developed countries are a rare but potentially intervenable cause of optic neuropathy in pediatric populations. To date, much of the literature on nutritional optic neuropathy has focused on children with developmental delay, however, a growing body of evidence supports other underreported risk factors.

Observations: We describe three pediatric patients with normal neurodevelopment, who presented with decreased vision and were subsequently found to have optic neuropathy attributed to vitamin deficiencies, predominantly vitamin B12.

Presumed bilateral acute idiopathic maculopathy in a pediatric patient.

Purpose: To describe a rare case of presumed bilateral acute idiopathic maculopathy (AIM) in a pediatric patient.

Observation: An 11-year-old male was evaluated for a "fuzzy Dorito-shaped" spot in the central vision of his right eye (OD) that started 3 days before presenting to our clinic. On examination, best-corrected visual acuity (BCVA) was counting fingers at 5 feet OD, and 20/25 in the left eye (OS).
