Most tumours of the tracheobronchial tree are malignant. Benign tumours such as hamartoma are infrequent and generally intra-parenchymal. Here we present a case of a 65-year-old male patient who presented with a purely endobronchial lobulated mass lesion in the left main bronchus. This central airway obstruction was managed by a complete endobronchial resection using an electrocautery snare and cryo-recanalisation techniques. A diagnosis of endobronchial chondroid hamartoma was made on histopathological examination. Such endobronchial lesions are rare and constitute less than 2% of all hamartomas.

We report a case of a 68-year-old gentleman, found to have a right hilar soft tissue mass whilst undergoing CT staging for prostate cancer. MRI imaging showed a heterogenous, enhancing solid mass without evidence of fat content. A linear probe endobronchial ultrasound-guided transbronchial needle aspiration was performed using a 19G needle.

Pulmonary chondromatous (or chondroid) hamartomas are common benign tumours of the lung and may be either parenchymal or endobronchial. The latter are only rarely encountered and have a variation in their clinical presentation related to the endobronchial obstructive lesion, including atelectasis, pneumonia, hemoptysis. If not diagnosed early and treated properly, endobronchial hamartomas can cause irreversible lung damage.
