Objectives: The quest to measure and improve diagnosis has proven challenging; new approaches are needed to better understand and measure key elements of the diagnostic process in clinical encounters. The aim of this study was to develop a tool assessing key elements of the diagnostic assessment process and apply it to a series of diagnostic encounters examining clinical notes and encounters' recorded transcripts. Additionally, we aimed to correlate and contextualise these findings with measures of encounter time and physician burnout.
Design: We audio-recorded encounters, reviewed their transcripts and associated them with their clinical notes and findings were correlated with concurrent Mini Z Worklife measures and physician burnout.
Setting: Three primary urgent-care settings.
Participants: We conducted in-depth evaluations of 28 clinical encounters delivered by seven physicians.
Results: Comparing encounter transcripts with clinical notes, in 24 of 28 (86%) there was high note/transcript concordance for the diagnostic elements on our tool. Reliably included elements were red flags (92% of notes/encounters), aetiologies (88%), likelihood/uncertainties (71%) and follow-up contingencies (71%), whereas psychosocial/contextual information (35%) and mentioning common pitfalls (7%) were often missing. In 22% of encounters, follow-up contingencies were in the note, but absent from the recorded encounter. There was a trend for higher burnout scores being associated with physicians less likely to address key diagnosis items, such as psychosocial history/context.
Conclusions: A new tool shows promise as a means of assessing key elements of diagnostic quality in clinical encounters. Work conditions and physician reactions appear to correlate with diagnostic behaviours. Future research should continue to assess relationships between time pressure and diagnostic quality.

High-entropy metal-organic frameworks (HE-MOFs) hold promise as versatile materials, yet current rare examples are confined to low-valence elements in the fourth period, constraining their design and optimization for diverse applications. Here, a novel high-entropy, defect-rich and small-sized (32 nm) UiO-66 (ZrHfCeSnTi HE-UiO-66) has been synthesized for the first time, leveraging increased configurational entropy to achieve high tolerance to doping with diverse metal ions. Due to the sulfur's atoms' propensity to form molecules and/or polymeric chains of various sizes and configuration, elemental sulfur possesses more allotropes and polymorphs than any other element at ambient conditions. This variability of the starting building blocks is partially responsible for its rich and fascinating phase diagram, with pressure and temperature changing the states of sulfur from insulating molecular rings and chains to semiconducting low- and high-density amorphous configurations to incommensurate superconducting metallic atomic phase. Here, using a fast compression technique, we demonstrate that the rapid pressurisation of liquid sulfur can effectively break the molecular ring structure, forming a glassy polymeric state of pure-chain molecules (Am-S).

Objective: This study compared the fracture load, stress distribution, and survival probability under cyclic loading of extensively restored teeth treated with multisonic irrigation with those treated with conventional instrumentation, with or without a post.
Methods: Mesial-occlusal-distal cavities were prepared in 30 human mandibular premolars. The teeth were randomly divided into 3 groups of 10 based on the endodontic and restorative procedures: (1) Root canal treatment (RCT) followed by resin composite restoration (control group), (2) RCT followed by a glass fiber post restoration (conventional group), and (3) minimal instrumentation plus multisonic irrigation followed by resin composite restoration (GW group).
