Background: Paliperidone is a second-generation antipsychotic and the main active metabolite of risperidone, formulated to provide consistent therapeutic effects through an extended-release system, designed to provide consistent therapeutic effects through an extended-release formulation. While commonly used in clinical practice, switching from risperidone to paliperidone, particularly during valproate therapy, can pose challenges due to potential pharmacokinetic interactions that may increase the risk of extrapyramidal symptoms (EPS). Despite clinical observations suggesting these interactions, case reports documenting such adverse effects are scarce.

Background: The overuse of antipsychotics in persons with dementia in long-term care (LTC) has been a source of clinical concern, public attention, and policy intervention for over 30 years. Targeted quality improvement, broader awareness of risks, and other initiatives have resulted in substantial reductions in antipsychotic use in LTC settings in North America and elsewhere. Limited evidence suggests that reductions in antipsychotic use may be resulting in unintended consequences, such as substitution with alternate, but similarly harmful, psychotropic medications.

The antipsychotic medication haloperidol has been used for many years in avian medicine as a pharmacologic therapy for refractory feather destructive behavior in pet parrots. However, despite its common use, there are no published studies evaluating its efficacy and adverse effects in psittacine birds. The goal of this study was to report the signalment, clinical presentation, dosing regimen, response to therapy, and adverse effects of companion psittacine birds prescribed oral haloperidol therapy at a single veterinary referral hospital.
