This is a case report of a 67-year-old man with the rare autoimmune disease relapsing polychondritis. The patient was initially diagnosed by general practitioners with erysipelas around his left ear, which was found red, swollen, and painful. Due to the lack of effect from antibiotics, the patient was referred to an emergency department. A rheumatologist recognised the patterns of the rare disease, diagnosed the patient and initiated proper treatment. The case clarifies the difficulty in diagnosing relapsing polychondritis, mainly due to the rarity and lack of knowledge of the disease.

This review is describing the diagnostic and therapeutic approach to tracheobronchial involvement in relapsing polychondritis (RP), with a focus on differential diagnoses of inflammatory origin. RP is a systemic auto-immune disease that mainly affects cartilage structures, progressing through inflammatory flare-ups between phases of remission and ultimately leading to deformation of the involved cartilages. Besides the damage of auricular or nasal cartilage, tracheobronchial and cardiac involvement are the most severe, and can seriously alter the prognosis.

Cutaneous vasculitis is frequently present in patients with systemic lupus erythematosus (SLE), rheumatoid arthritis, and Sjögren's syndrome and is less common in dermatomyositis, systemic sclerosis, and relapsing polychondritis. Given the low prevalence of vasculitic involvement in this disease, the skin lesions described so far have had minimal semiological characterization. This has limited the provision of clear guidelines regarding their treatment, with most therapeutic interventions being extrapolated from idiopathic chronic leukocytoclastic vasculitis.
