As part of a routine procedure, our laboratory frequently encounters questions from clients/Medical Review Officers (MROs) regarding opioid hair concentrations in relation to the amount of opioid taken as part of a prescription. In this manuscript, we have analyzed a large number of real-world examples of opioid hair concentrations following self-reported consumption of an opioid prescription regimen.

The opioid crisis has been an issue in the United States since the mid-1990s, claiming numerous lives and presenting a significant challenge to health care clinicians. Various preoperative, intraoperative, and postoperative strategies aimed at reducing opioid consumption can be used by orthopaedic surgeons to help minimize this crisis. This article presents the case of a 67-year-old African American male patient who was referred to a psychiatry clinic by his Internal Medicine Provider with a diagnosis of "schizophrenia, unspecified." The patient reported the onset of auditory and visual hallucinations (AVHs) two years ago, coinciding with his starting Norco (hydrocodone 5 mg/acetaminophen 325 mg) for chronic back pain. He noted that his AVH worsened when he increased his prescribed Norco dosage (within his prescribed recommended range) and observed that the hallucinations ceased when he discontinued the medication.
