Massive and torrential tricuspid regurgitation may be accompanied by very large gaps, posing a challenge for transcatheter edge-to-edge repair (TEER). These 2 cases involve patients who underwent successful TEER despite very large gaps. Both patients experienced significant improvement after TEER. Feasibility and challenges of TEER in very large gaps are presented. ().
