Introduction And Importance: The case report highlights the challenges in managing recurrent Mooren's ulcer, a rare and severe form of peripheral ulcerative keratitis.

Case Presentation: A 49-year-old male, with bilateral Mooren's disease who underwent multiple surgical interventions for visual rehabilitation but developed disease recurrence in the left eye despite being on systemic and topical immunosuppression. The authors report successful long-term visual rehabilitation following a large-graft penetrating keratoplasty and Ahmed glaucoma valve implantation performed under immunosuppressive cover after adequate control of inflammation with intravenous Rituximab.

Clinical Discussion: We discuss the traditionally poor survival of penetrating keratoplasty in refractory Mooren's disease due to the recurrence of the disease in the graft. Occurrence of secondary glaucoma is commonly seen in these eyes, which often requires combined filtration surgery and medical management. The first and second line systemic immunosuppression may be insufficient in adequately controlling the disease, and that intravenous monoclonal antibodies, such as Rituximab, may be a promising treatment modality in refractory Mooren's disease prior to embarking on any surgical intervention.

Conclusion: Overall, the case report highlights the importance of adequate control of inflammation and the use of immunosuppressive cover in achieving successful outcomes of PK and AGV surgery in Mooren's ulcer. The authors suggest that intravenous Rituximab may be a valuable addition to the management of refractory Mooren's disease, particularly prior to surgical intervention. However, as with any case report, the findings should be interpreted with caution and further research is needed to validate the efficacy of this treatment modality.

Background: Conjunctival sporotrichosis is a rare fungal infection, typically presenting as granulomatous lesions. Its manifestations can be atypical, particularly in immunosuppressed patients. Here, we present a rare case of a Mooren's ulcer patient with bulbar conjunctival Sporotrichosis presenting as a salmon-pink tumor.

Risk factors for recurrence of Mooren's ulcer after keratoplasty.

Xiamen Eye Center and Eye Institute of Xiamen University, School of Medicine, Xiamen, China.

Purpose: The purpose of this study was to investigate the clinical characteristics of patients with corneal Mooren's ulcers (MU) and explore potential risk factors for recurrence after keratoplasty.

Methods: The study retrospectively analyzed 87 patients (101 eyes) diagnosed with MU. Factors associated with recurrence after keratoplasty were identified using correlation analysis, and a clinical scoring system was developed based on the magnitude of the univariate and multivariate logistic regression analysis.

Perspectives of Patients and Clinicians on Reproductive Health Care and ADPKD.

Kidney Int Rep

November 2024

Department of Obstetrics and Gynecology, Wilhelmina Children's Hospital Birth Centre, University Medical Centre Utrecht, Utrecht, the Netherlands.

Introduction: Family planning and reproductive care are essential but complex aspects of lifecycle management for individuals with autosomal dominant polycystic kidney disease (ADPKD), given the potential genetic transmission and pregnancy-related complications. In this qualitative study, we studied the experiences and perspectives of patients with ADPKD and clinicians to identify areas for potential improvement in reproductive lifecycle care.

Methods: Focus group discussions (FGDs) were conducted in the Netherlands with patients with ADPKD, both men and women, who had children through varied reproductive choices; and clinicians, including (pediatric) nephrologists, obstetric gynecologists and geneticists.

Mooren's ulcer is a painless and idiopathic ulcer of the peripheral cornea related to autoimmunity against a corneal stromal antigen, calgranulin C. Corneal involvement is isolated. There are no specific histopathological features to differentiate Mooren's ulcer from pseudo-Mooren's, the latter being part of a systemic disease.
