Acute kidney injury (AKI) is typically classified as prerenal, renal, or postrenal in etiology, with postrenal often referring to obstructive causes. However, certain uncommon conditions, such as intraperitoneal urinary leaks, may not fit clearly into these categories. In patients with a recent history of pelvic procedure, a complication such as intraperitoneal urinary leak can mimic AKI due to urine reabsorption across the peritoneum.

Spontaneous urinomas are rare, encapsulated urine collections in the retroperitoneum that occur without prior trauma or obstruction. It often presents with nonspecific abdominal symptoms, mimicking various acute abdominal pathologies. This case report describes a rare and unique instance of spontaneous urinoma secondary to pyeloureteritis, presenting with atypical clinical features that mimic acute bowel obstruction.

Lymphoscintigraphy is an established modality for imaging the lymphatic system using radiocolloids and is routinely indicated to find the cause of limb lymphedema. However, in this case series, we are highlighting other less-known indications of lymphoscintigraphy like chylothorax and chyluria which present as lymphatic leaks in the thorax and abdomen, respectively. Once the site of the lymphatic leak is established by lymphoscintigraphy, definitive management like thoracic duct ligation or sclerotherapy can be done.
