Published: December 2022

Congenital transmesenteric defect is a rare but serious condition which presents a diagnostic challenge as symptoms and investigations are nonspecific. Here, we present a case with fetal ascites and dilated bowel loops who underwent intervention within 6 h of birth. Laparotomy showed volvulus and gangrene of 45 cm of the distal ileum, which was herniating through mesenteric defect. Timely detection and intervention can help in preventing significant morbidity and mortality. Given the downside to delay of surgical intervention and lack of specific tests, open exploration continues to be the only way to establish a definitive diagnosis of a transmesenteric hernia.

Background: Small mesenteric hiatal hernias (SMHHs) are defined as a small group of internal hernias (IHs) that frequently diagnosed in children. However, SMHHs are relatively rare in adults. Bowel loop herniation via an abnormal mesenteric defect can lead to strangulated intestinal obstruction.

Article Synopsis
  • - Transmesentric hernias involve the bowel looping through a defect in the mesentery and can cause small bowel obstruction, leading to high mortality due to delayed diagnosis.
  • - A case series detailed four patients, including three children and one preterm neonate, all presenting with bowel obstruction symptoms and requiring emergency surgery to address the hernia and remove gangrenous bowel.
  • - Timely surgical intervention is crucial, as traditional diagnostic methods may not clearly identify this rare condition, making it essential to consider transmesentric hernias in differential diagnoses for pediatric bowel obstructions.
