Background: Ischemia-free liver transplantation (IFLT) has been innovated to avoid graft ischemia during organ procurement, preservation, and implantation. However, the metabolism activity of the donor livers between in the in situ and ex situ normothermic machine perfusion (NMP) conditions, and between standard criteria donor and extend criteria donor remains unknown.
Methods: During IFLT, plasma samples were collected both at the portal vein and hepatic vein of the donor livers in situ during procurement and ex situ during NMP. An ultra-high performance liquid chromatography-mass spectrometry was conducted to investigate the common and distinct intraliver metabolite exchange.
Results: Profound cysteine and methionine metabolism, and aminoacyl-tRNA biosynthesis were found in both in situ and ex situ conditions. However, obvious D-arginine and D-ornithine metabolism, arginine and proline metabolism were only found in the in situ condition. The suppressed activities of the urea cycle pathway during ex situ condition were confirmed in an RNA expression level. In addition, compared with extend criteria donor group, standard criteria donor group had more active intraliver metabolite exchange in metabonomics level. Furthermore, we found that the relative concentration of p-cresol, allocystathionine, L-prolyl-L-proline in the ex situ group was strongly correlated with peak alanine aminotransferase and aspartate aminotransferase at postoperative days 1-7.
Conclusions: In the current study, we show the common and distinct metabolism activities during IFLT. These findings might provide insights on how to modify the design of NMP device, improve the perfusate components, and redefine the criteria of graft viability.

Background: Patients with end-stage renal disease often face prolonged waiting times for kidney transplants. Historically, the use of marginal kidneys was limited due to suboptimal preservation methods. Normothermic machine perfusion (NMP) preserves physiological activity during the preservation process, potentially improving graft function and viability, expanding the use of marginal kidneys.

Background: Multiple intraoperative hemodynamic parameters are associated with an increased risk of early allograft dysfunction (EAD) following living donor liver transplantation (LDLT); however, there is significant center-to-center variability in terms of which parameters are used. We sought to determine which intraoperative hemodynamic parameters are most predictive of EAD following LDLT.
Methods: This is a systematic review following PRISMA guidelines (PROSPERO ID: CRD42023409711).

Background: Digital nerve injuries significantly affect hand function and quality of life, necessitating effective reconstruction strategies. Autologous nerve grafting remains the gold standard due to its superior biocompatibility, despite recent advancements in nerve conduits and allogenic grafts. This study aims to propose a novel zone-based strategy for donor nerve selection to improve outcomes in digital nerve reconstruction.

Background: Rapid, reproducible, and noninvasive diagnostic methods like Ultrasonography (US) and plethysmographic measurements such as the perfusion index (PI) and pleth variablity index (PVI) have great potential value for emergency trauma cases in which blood loss needs to be recognized quickly and accurately.
Objectives: We planned this study to evaluate the utility of US, PI, and PVI in detecting early-stage hemorrhage and mimicking volume replacement using a platelet apheresis model.
Methods: This prospective, observational study included 46 healthy platelet apheresis volunteers who met inclusion criteria.

Tooth autotransplantation is widely used to replace congenitally missing teeth or teeth with irreversible damage. This case report presents a personalized ultrasonic osteotome that enables precise preparation, minimizes bone trauma, enhances the initial stability of the transplanted tooth, and contributes to a favorable prognosis. The procedure is as follows: a 25-year-old female patient presented with a porcelain-fused-to-metal crown on Tooth #19, which had detached due to severe decay, rendering the tooth unsalvageable.
