Study Design: Retrospective observational study.

Objective: The objective of this study was to investigate factors associated with cervical kyphosis after laminoplasty.

Summary Of Background Data: Many factors are reportedly associated with the deterioration of cervical curvature after laminoplasty, including cervical lordosis angle, cervical spine range of motion (ROM), T1 slope, and C2-C7 sagittal vertical axis. Postlaminoplasty kyphosis or deterioration of cervical curvature is likely caused by multiple factors. There is currently no consensus on these issues.

Materials And Methods: Data of patients treated with laminoplasty for degenerative cervical myelopathy at our institution during 2008-2018 were reviewed. The following variables were collected for each patient: age and sex; follow-up time; surgery involving C3 (yes or no); surgery involving C7 (yes or no); distribution of segments operated on; number of laminae operated on; flexion, extension, and total ROM; cervical lordotic angle; longitudinal distance index; curvature index; C2-C7 sagittal vertical axis; and T1 slope. Logistic regression analysis was used to assess possible risk factors for postoperative kyphosis. Receiver operating characteristic curves were constructed to determine the cutoff values of risk factors.

Results: The study cohort comprised 151 patients. Logistic regression analysis indicated that sex, number of laminae operated on, and preoperative extension ROM were significantly associated with postoperative cervical kyphosis ( P <0.05). There was significantly greater postoperative kyphosis in women than in men; the more segments operated on, the greater the risk of postoperative kyphosis, and the larger the preoperative extension ROM, the lower the risk of postlaminoplasty kyphosis. Receiver operating characteristic curve analysis showed that the cutoff value for preoperative extension ROM is 22.1°.

Conclusions: Preoperative extension ROM may be associated with the development of postoperative kyphosis. The cutoff value of preoperative extension ROM that suggested the prospect of postoperative kyphosis in our sample was 22.1°.

Keyhole decompression surgery for holospinal epidural abscess: a novel approach for spinal stability preservation.

Eur Spine J

January 2025

Department of Orthopaedic Surgery, Graduate School of Medicine, Chiba University, 1-8-1 Inohana, Chuo-ku, Chiba, 260-8670, Japan.

Purpose: Spinal epidural abscesses are rare yet serious conditions, often necessitating emergency surgical intervention. Holospinal epidural abscesses (HEA) extending from the cervical to the lumbosacral spine are even rarer and present significant challenges in management. This report aims to describe a case of HEA with both ventrally-located cervical and dorsally-located thoracolumbar epidural abscesses treated with a combination of anterior keyhole decompression and posterior skip decompression surgeries.

: Multilevel cervical spondylotic myelopathy (MCSM) presents complex challenges for surgical management, particularly in patients with kyphosis or significant anterior pathology. This study aimed to assess the long-term efficacy of modified expansive open-door laminoplasty (MEOLP) combined with short-level anterior cervical fusion (ACF) in providing decompression, preserving alignment, and maintaining range of motion (ROM) over a nine-year follow-up. : A retrospective analysis was conducted on 124 MCSM patients treated with MEOLP combined with ACF between 2011 and 2015.

Lumbopelvic pain (LBP) is a prevalent condition during pregnancy, affecting a significant proportion of pregnant women. It arises from hormonal, biomechanical, and postural changes, often exacerbating discomfort and impairing quality of life. This study aimed to evaluate the effects of targeted motor control interventions focusing on sternal alignment on spinal alignment, pain, and muscle activity in pregnant women at risk of preterm birth.

High Preoperative T1 Slope is a Marker for Global Sagittal Malalignment.

Clin Spine Surg

January 2025

Department of Orthopedic Surgery, NYU Langone Health, New York, NY.

Article Synopsis
  • The study analyzed preoperative data from adult spinal deformity patients to establish a threshold for T1 slope (T1S) from cervical radiographs that indicates thoracolumbar malalignment.
  • Researchers found a specific cutoff of 30 degrees for high T1S, with 50% of the patients falling above this threshold.
  • Notably, higher T1S was linked with greater thoracic kyphosis, sagittal vertical axis, T1-pelvic angle, and pelvic tilt, suggesting that this measurement could serve as a useful parameter in cervical evaluations.

To analyze the influence of cervical range of motion on the preferable sagittal vertical axis in ankylosis spondylitis (AS)-related thoracolumbar kyphosis following single-level pedicle subtraction osteotomy (PSO). The clinical data of sixty-five AS patients who underwent single-level PSO from February 2012 to November 2018 in the Drum Tower Hospital of Nanjing University Medical School were retrospectively reviewed. Of the patients, 59 were males and 6 were females with a mean age of (34.
