Particularly severe cases with tunneled defects are rarely reported and are described only in a few case reports. This case report describes the treatment of a tunnel fenestration in the lower central jaw after unsuccessful endodontic treatment following trauma of incisors 31 and 41 over the course of six years, which led to the development of an internal granuloma and a radicular cyst in the lower jaw. The patient presented with a 2.67 cm radicular cyst displacing the surrounding tissue at regio 31 and 41, which resulted in a tunnel-like bony defect. Endodontic treatment and periapical root tip resection on teeth 31 and 41 with cystectomy, and with a 12 month follow-up, were successful in the healing of the bone defect. The preserved teeth received lithium disilicate crowns for definite restoration one year postoperatively. This treatment can be an option for the therapy of large cysts.

A novel surgical technique to resolve mucosal fenestration of a root apex: Apical tunnel surgery: A case report.

Medicine (Baltimore)

July 2024

Department of Periodontics, Stomatological Hospital, School of Stomatology, Southern Medical University, Guangzhou, P. R. China.

Rationale: Endodontic surgery, which includes apex resection, retro-fill and some regeneration procedures, is a traditional way to deal with apex fenestration. The endodontic surgery could bring large flap, curtate root length, non-healing mucosa and soft tissue deficiency in the apex area. Other treatment options might be considered according to different etiological factors.

Background: Even though the optimal management of a moderate or large residual shunt following patent foramen ovale (PFO) closure is open to question, recent data confirmed that it is associated with an increased risk of stroke recurrence.

Case Summary: A 48-year-old woman, a migraineur with visual aura, was diagnosed with a PFO associated with a huge multifenestrated atrial septal aneurysm (mfASA) and a moderate right-to-left shunt, detectable only after a Valsalva maneuver on contrast-transthoracic echocardiography. Brain magnetic resonance imaging showed a 1-mm silent white matter lesion in the right frontal lobe.

Patients with functionally univentricular hearts are usually palliated surgically. There have been several reports of successful attempts to complete the Fontan procedure without surgery. The pathways created at the time of the preconditioning were largely reminiscent of the lateral tunnel Fontan.

Background: Utilization of Fontan fenestration varies considerably by center.

Objectives: Using a multicenter Pediatric Heart Network dataset linking surgical and preoperative hemodynamic variables, the authors evaluated factors associated with use of Fontan fenestration and the impact of fenestration on post-Fontan length of stay (LOS).

Methods: Patients 2 to 6 years old at Fontan surgery from 2010 to 2020 with catheterization<1 year prior were included.
