An 11-year-old boy was admitted to our hospital due to severe pain in his right knee when he landed after jumping over a vaulting box. A plain X-ray image and computed tomography scan showed an avulsion fracture of the lower pole of the patella and patella alta. Furthermore, magnetic resonance imaging (MRI) revealed an articular cartilage lesion and rupture between the inferior pole of the patella and the patella tendon. We diagnosed a sleeve fracture of the patella and performed surgical treatment. Open reduction and internal fixation were performed by the pull-out technique using transosseous no. 2 MaxBraid™​​​​​​ ​(Zimmer Biomet, Tokyo, Japan) sutures. While postoperative weight-bearing was permitted, the knee joint was immobilized in a brace for four weeks. Three months of postoperative assessment revealed excellent functional outcomes.

Instruction-induced modulation of the visual stream during gesture observation.

Although gesture observation tasks are believed to invariably activate the action-observation network (AON), we investigated whether the activation of different cognitive mechanisms when processing identical stimuli with different explicit instructions modulates AON activations. Accordingly, 24 healthy right-handed individuals observed gestures and they processed both the actor's moved hand (hand laterality judgment task, HT) and the meaning of the actor's gesture (meaning task, MT). The main brain-level result was that the HT (vs MT) differentially activated the left and right precuneus, the left inferior parietal lobe, the left and right superior parietal lobe, the middle frontal gyri bilaterally and the left precentral gyrus.

Background: The anterior oblique bundle of the medial ulnar collateral ligament (UCL) inserts on the anteroinferior aspect of the humeral medial epicondyle, while the flexor pronator mass (FPM) originates superficial and proximal to the UCL. With valgus stress, these distinct footprints may produce injury patterns that affect only focal areas of the medial epicondyle.

Hypothesis: The proximal UCL can act on the medial epicondyle either in isolation or in conjunction with the FPM to form partial avulsion fracture patterns within the pediatric medial epicondyle, and the predominant pattern involves only the proximal UCL footprint.
