The National Institutes of Health (NIH) R01 grant represents the gold standard for research independence across the United States. The barrier to entry for this grant mechanism is extremely high, with <10% of grants being funded. Interestingly, after this tremendously high barrier, there is very little accountability for the grantee to accomplish the aims that were originally proposed. While there is certainly value to requiring investigators to provide a logical and compelling research plan, excessive grant writing creates a large inefficiency. Thus, a "fail quickly" model, which is not without cost, should be considered. Such a model involves a much lower barrier to entry and much higher short-term accountability so that more aims can be tested in less time at lower cost.

Background: Hemophagocytic lymphohistiocytosis (HLH), is characterized by systemic uncontrolled inflammation resulting from immune dysregulation secondary to various triggers, including genetics, infections, autoimmune diseases, and malignancies. Macrophage activation syndrome (MAS) is an immune dysregulation phenomenon, in which an underlying rheumatological disease is present. We report a rare, interesting case of a middle-aged female, with a systemic lupus erythematosus (SLE) flare complicated by macrophage activation syndrome (MAS), in which tuberculous meningitis (TBM) was the identified trigger.

Introduction: While various demographic factors and underlying medical conditions are associated with the development of post-COVID conditions within a month after SARS-CoV-2 infection, less is known about factors associated with post-COVID symptoms that persist for 6 months or more. The aim of this review was to determine the association between underlying conditions, other risk factors, health behaviors, and the presence of symptoms ≥6 months after COVID-19.
Methods: Studies reporting on post-COVID symptoms were searched in databases, including Medline, EMBASE, Global Health, PsycInfo, Scopus, CINAHL, Proquest, and WHO COVID-19 literature, from the beginning of the pandemic until November 2022.
