Abnormal fistulization is a rare complication of Nissen fundoplication. A high index of suspicion and timely diagnosis are key factors to guide appropriate management. We describe a 70-year-old woman with a remote history of Collis gastroplasty and Nissen fundoplication presenting with chest pain and a pericardial effusion.

A 58-year-old woman underwent an extended thymectomy and left upper lobectomy subsequent to induction chemoradiotherapy for advanced thymic carcinoma. The pathologic stage of the tumor was ypT0 N0 M0 stage 0. Six months after surgery, she presented with edema in her left arm and left breast.

A 68-year-old woman who had a retrievable inferior vena cava filter implanted 10 years ago presented with sudden-onset back pain. Initial computed tomography angiography demonstrated migration of a fractured strut that appeared embedded in the anterior right ventricular free wall without pericardial effusion. Subsequent gated computed tomography of the chest demonstrated further migration of the fragment, which was now penetrating the right ventricular free wall and extending into the pericardial sac.
