A patient is reported who had undergone right adrenalectomy for pheochromocytoma and 15 yr later developed a recurrence in the same site complicated by the watery diarrhea, hypokalemia, achlorhydria syndrome. This tumor was histologically defined as a composite malignant pheochromocytoma-ganglioneuroblastoma (well differentiated type). Vasoactive intestinal polypeptide and catecholamine concentrations were elevated in both plasma and the tumor. The tumor somatostatin content also was high. The tumor was immunohistochemically determined to contain both vasoactive intestinal polypeptide and somatostatin.

Cerebral palsy (CP) is a debilitating disorder that can lead to life-long disability, with a high incidence in Saudi Arabia. Secondary epilepsy and cardiac complications are common in CP patients. We present a rare case of a 17-year-old female with CP, attention-deficit hyperactivity disorder (ADHD), secondary epilepsy, and a history of post-cardiac arrest, with home medications carbamazepine, risperidone, and sodium valproate.

Anamnesis And Clinical Examination: A 59-year-old female patient with an unremarkable medical history presented with watery diarrhea, severe lower abdominal pain, and fever for approximately 7 days. Physical examination revealed a distended abdomen, bowel sounds were absent, and there was an acute abdomen with generalized guarding and significantly elevated inflammatory markers.
Diagnosis: Abdominal computed tomography showed significant ascites and signs of peritonitis.
