The left subphrenic crescent sign in direct cholangiography.

Published: September 1987

Left subdiaphragmatic collections of contrast medium were demonstrated in three patients during cholangiography. All the patients had undergone previous surgery for either biliary, gallbladder or pancreatic disease. The shape of the collections was crescentic and the underlying abnormality was an anterior subdiaphragmatic abscess. Left subphrenic abscesses should therefore be considered when crescentic left subphrenic accumulations of contrast medium occur during cholangiography.

Open surgical treatment of idiopathic chylothorax via thoracic duct ligation and pericardiectomy requires a lengthy procedure with two thoracotomy incisions. The objectives of this report were to describe an approach for thoracic duct ligation and pericardiectomy via a single thoracotomy at the left fourth intercostal space and to describe the clinical outcome in two dogs with idiopathic chylothorax. Dogs were prospectively enrolled in a pilot study to evaluate the clinical efficacy of thoracic duct ligation at the left fourth intercostal space, combined with subphrenic pericardiectomy performed through the same incision.

Introduction: Intraabdominal collections (IACs) are common following cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC). Inflammatory biomarkers and nutritional index (NI) may predict IACs. Management of IACs with image guided percutaneous drainage (IGPD) is an alternative to laparotomy.

Article Synopsis
  • - Subdiaphragmatic abscess drainage can be challenging and may lead to complications like organ damage and infection spread.
  • - A new technique, EUS-guided drainage, has been successfully used for treating upper abdominal abscesses, including a case involving a subdiaphragmatic cyst from a perforated sigmoid colon after ovarian cancer surgery.
  • - The case involved a 60-year-old Japanese woman who benefited from this minimally invasive procedure, which led to the cyst's reduction without causing additional complications.

Objective: To investigate a left-sided fourth intercostal approach to thoracic duct (TD) ligation and unilateral subphrenic pericardiectomy in dogs.

Study Design: Retrospective computed tomography (CT) review and cadaveric study.

Animals: Thirteen dogs with idiopathic chylothorax and 10 canine cadavers.

Chilaiditi's sign refers to colonic interposition between the liver and the diaphragm in the right subphrenic space secondary to the relaxation of the suspensory ligaments of the right colic flexure. The diagnosis of Chilaiditi's sign is based on radiological findings with the following three criteria: 1) The right hemidiaphragm must be adequately elevated above the liver by the intestine, 2) the bowel must be distended by air to illustrate pseudo-pneumoperitoneum, and 3) the superior margin of the liver must be depressed below the level of the left hemidiaphragm. In this report, we present the case of a 49-year-old female presenting with signs and symptoms suggestive of Chilaiditi syndrome managed with laparoscopic surgery.
