Multisystem Inflammatory Syndrome in Children (MIS-C) is a rare febrile disorder with multisystem organ involvement temporally associated with coronavirus 2019 infection (COVID-19) and frequently exhibits features mimicking Kawasaki disease (KD), another febrile disorder in children. The pathogenesis and the full clinical spectrum of MIS-C is poorly understood: It is still unclear whether MIS-C and KD are different syndromes or represent a common spectrum. The erythema and induration of Bacillus Calmette-Guérin (BCG) scar is one of the characteristic findings of KD, and is useful for the diagnosis in countries where BCG vaccination is mandated in infancy.

Intraosseous meningioma mimicking osteosarcoma.

Autops Case Rep

October 2021

Mount Sinai Medical Center, The Arkadi M. Rywlin M.D. Department of Pathology and Laboratory Medicine, Miami Beach, FL, USA.

Background: Predominantly intraosseous meningiomas are rare entities that include true primary intraosseous meningiomas (PIM), as well as meningiomas that may show extensive bone involvement, such as meningiomas. Different hypotheses have been proposed to decipher the origin of PIMs, such as ectopic arachnoid cap cell entrapment during birth or after trauma. Surgical resection is the treatment of choice of such lesions.

Introduction: A novel hyperinflammatory syndrome has emerged in the paediatric population: paediatric inflammatory multisystem syndrome - temporally associated with SARS-CoV-2 (PIMS-TS). Up to 50% of patients develop shock with cardiac dysfunction but presentation with acute abdominal pain is common and difficult to distinguish from appendicitis.

Method: Prospective case series of PIMS-TS patients presenting to a single UK tertiary paediatric centre.

Article Synopsis
  • - Most children with SARS-CoV-2 have mild or no symptoms, but a small group develops a serious inflammatory condition known as MIS-C, which resembles other severe immune responses.
  • - A case study describes a boy with MIS-C who showed unusual lab results and ultimately suffered from severe complications, including fatal liver failure linked to EBV-associated HLH, despite receiving multiple treatments.
  • - The diagnosis of X-linked lymphoproliferative disease type 1 (XLP1) post-mortem suggests that doctors should consider HLH in severe MIS-C cases for better treatment strategies and genetic counseling.
