We report the case of an intraosseous needle used to drain an acute extra-dural haematoma in a remote hospital. An 18 yr. old female attended the Emergency Department, after sustaining a closed head injury from a fall. After a CT scan, she was diagnosed with a large acute extradural haematoma (EDH). Prior to air ambulance transfer to the Neurosurgical Centre, she developed a fixed dilated pupil and hemodynamic instability. The Neurosurgeon advised that an intraosseous (IO) needle drainage would prevent brain stem herniation. An Emergency Medicine (EM) consultant drained 60 ml of blood and clot via an IO needle. The pupil and cardiovascular status normalised. The patient underwent neurosurgical drainage with full neurological recovery. We believe that this is the first IO drainage of an EDH at a remote hospital followed by full neurological recovery.

Background: To demonstrate the safety and performance of the Arrow EZ-IO Intraosseous Vascular Access System, particularly in the pediatric patient population, a retrospective observational study was conducted in 2021 and 2022.
Methods: Following study design, IRB approval, and investigator selection, data were collected for all patients needing intraosseous access-adult and pediatric. The primary endpoint was the success rate for achieving intraosseous access; the secondary endpoint was the rate of adverse events.

Introduction: Intraosseous (IO) access is a commonly used procedure in pediatric emergencies for establishingvascular access when alternative means, such as intravenous access, cannot be obtained. Likealternative routes, IO can be administered quickly and provides a route for medications, blood products,and fluids to be infused in emergency circumstances. While the use of IO is generally considered safewith minimal risks, potential complications can include compartment syndrome, infection, thrombosis, and fracture.

Intraosseous hemangiomas are benign vascular tumors which are more commonly seen in the vertebrae and skull but they rare in the ribs. Rib hemangiomas can mimic malignant rib lesions and definitive clinical and imaging diagnosis might be difficult and pathological confirmation will be required. Here we report a rib hemangioma in a 49-year-old male presented with painless chest wall swelling of 9 years duration, on chest CT scan the lesion showed dense radiating type calcification with cortical destruction.
