Introduction: Endolymphatic sac tumor (ELST) is a rare neoplasm that exhibits aggressive growth primarily in the endolymphatic capsule and can potentially affect nearby neurovascular structures. The diagnosis of ELST poses challenges due to its low prevalence, gradual progression, and nonspecific symptomatology. Background: Dermatomyositis is a chronic inflammatory condition affecting muscles and skin, often associated with an increased risk of cancer. Specific autoantibodies, including anti-TIF1 (Transcription Intermediary Factor 1), have been linked to this risk. We present a case of dermatomyositis in a male patient positive for anti-TIF1 antibodies, subsequently diagnosed with squamous cell carcinoma of the tonsil, a novel association not previously documented.

Subungual melanoma is a variant of acral lentiginous melanoma that arises from the nail matrix. Subungual melanomas present unique clinical challenges due to diagnostic difficulties and the lack of a standardized protocol for surveillance, also, there are no evidence-based studies that determine the ideal frequency and duration of clinical and dermoscopy follow-ups in patients with longitudinal melanonychia. This is highlighted by a case of longitudinal melanonychia in a 53-year-old patient who underwent malignant transformation to subungual melanoma after a biphasic growth.

Pleural effusion as an initial presentation of malignancy poses significant diagnostic challenges, particularly when linked to gynecologic cancers. We discuss the case of a 53-year-old female who presented with progressive dyspnea and a massive right-sided pleural effusion. Cytological analysis of the pleural fluid revealed malignant cells and immunohistochemical staining confirmed high-grade serous carcinoma (HGSC) of ovarian origin.
