Objective: To investigate whether lateral lymph node (LLN) dissection (LLND) can be exempted in patients with good tumor regression grading (TRG) after neoadjuvant chemoradiotherapy (nCRT)?

Methods: A retrospective study was conducted on consecutive patients with advanced rectal cancer who underwent nCRT and total mesorectal resection plus selective LLND at our institution. The primary outcomes are the relationship between LLN metastasis (LLNM) and magnetic resonance imaging TRG (mrTRG) and the relationship between LLNM and pathological TRG (pTRG).

Results: A total of 91 patients were included, of which 24 patients (26.4%) had LLNM, 67 patients (73.6%) had no LLNM. There were significant differences of the maximum short-axis of LLN before and after nCRT, short-axis reduction rate of the LLN with maximum short-axis, length diameter reduction rate of primary tumor, mrTRG, and pTRG between the two groups. Multivariate logistic regression showed that mrTRG (P = 0.026) and pTRG (P = 0.013) were independent predictors for LLNM. The combination used by mrTRG and the maximum short-axis of LLNs ≥ 8 mm before nCRT and the maximum short-axis of LLN ≥ 5 mm after nCRT achieved specificity of 0.970, positive predictive value (PPV) of 0.867, and negative predictive value (NPV) of 0.855. The same combination used by pTRG achieved the specificity of 0.970, PPV of 0.857 and NPV of 0.844.

Conclusion: The suspected positive LLNs tend to be sterilized by nCRT in patients who have a very good response to nCRT. It is rational to avoid LLND in patients whose primary tumor and LLNs both show good response to nCRT.

Introduction: The objective of this study is to estimate reference values for medial retropharyngeal lymph nodes (MRLNs) measured in high-field (3T) MRI studies of the canine head/brain using transverse T2 spin-echo images and to determine if dogs with structural brain disease exhibit medial retropharyngeal lymph nodes that are larger than expected from estimated reference values.

Methods: The study population comprises 142 MRLNs from 71 dogs with no evidence of structural brain disease and normal CSF evaluation and 116 MRLNs from 58 dogs with structural brain disease confirmed by histopathology as of infectious or neoplastic origin, or to represent meningoencephalitis of unknown etiology.

Results: Based on this sample, MRLNs are expected to measure 2.

Purpose: Periprosthetic joint infection (PJI) is a severe complication following joint replacement surgery, and there is a current lack of rapid, accurate, and non-invasive diagnostic methods. This study aims to assess the effectiveness of using dual-energy computed tomography (DECT) to quantify the maximum long and short axes of the external iliac lymph nodes (LNs) as a diagnostic tool for PJI.

Patients And Methods: In this prospective controlled study, 112 patients undergoing total hip or total knee revision surgery were enrolled.

Background: Endoscopic sphincterotomy (ES) for bile duct stones (BDS) can cause basket impaction with stone, complicating the procedure. In this study, we evaluated the utility and safety of small incision ES combined with endoscopic papillary balloon dilation (sES + EPBD) (balloon dilated up to the stone size), compared with ES alone for BDS <12 mm.

Methods: The primary endpoint was the frequency of mechanical lithotripsy (ML), indicating the risk of basket impaction with stone; however, the secondary endpoints were procedure time, successful stone removal, and early adverse events.

Article Synopsis
  • The study investigates the efficacy of the Node Reporting and Data System 1.0 (Node-RADS) for diagnosing lymph node metastasis in patients with esophageal squamous cell carcinoma (ESCC).
  • Node-RADS uses a scoring system from 1 to 5 for evaluating lymph nodes, with a retrospective analysis of 173 ESCC patients who underwent CT scans before surgery.
  • Results show Node-RADS has high accuracy (AUC: 94.3%) and outperforms traditional assessment methods, providing an effective tool for identifying high-risk metastatic lymph nodes and guiding treatment decisions.

Background: This study aimed to optimize a lateral transthyroid approach by using high-resolution ultrasonography (HRUS) for recurrent laryngeal nerve (RLN) visualization.

Patients And Methods: In this prospective study of 85 patients undergoing thyroidectomy, the RLN was visualized preoperatively by using a lateral transthyroid approach via HRUS. The inferior thyroid artery, thyroid nodule, and cricoid cartilage were used as landmarks.
