CORONAL PLANE GROWTH MODULATION FOR GENU VALGUM IN SKELETAL DYSPLASIA.

Published: December 2022

Objective: To investigate the efficiency and rates of correction by hemiepiphysiodesis using 8-plate to manage genu valgum deformity in children with skeletal dysplasia.

Methods: Eleven children with skeletal dysplasia (three female, eight male; mean age = 10.5 years; age range = 7-13) who underwent temporary hemiepiphysiodesis using 8-plates for genu valgum deformity were retrospectively reviewed. There were nine bilateral cases and two unilateral cases. The mean follow-up time from the index surgery to the final follow-up was 45 (ranging from 24 to 72) months. Radiographical assessment including preoperative and final follow-up measurements of joint orientation angles and mechanical axis deviation (MAD) were conducted.

Results: Deformities were completely corrected in nine lower extremities (45%) and partially corrected in seven extremities (35%). In four extremities of two children with Morquio syndrome, MAD worsened. The correction rate of MAD was 1.25 ± 1.62 mm/mo.

Conclusion: Though hemiepiphysiodesis using 8-plate requires a longer treatment period, it seems to be an effective treatment for correction of genu valgum in children with skeletal dysplasia.

Primary hyperparathyroidism (PHPT) is relatively uncommon in children, and skeletal deformities due to it are even rarer in this age group. Less than 20 such cases have been reported in the English literature. We describe a case series of three patients who presented with genu valgum deformity and were found to have primary hyperparathyroidism on further evaluation.

Purpose Of The Study: Temporary hemiepiphyseodesis using figure-eight plates is currently one of the main surgical techniques to correct axial deformities of lower limbs in paediatric patients. Comprehensive analysis, correct indication and monitoring of treatment are the basic prerequisites for successful therapy. The aim of the study was to analyse parameters that could become an alternative to standard parameters used nowadays, namely the inserted screw angle (SA), and a new parameter - condylar ratio (CR).

Postaxial polydactyly (PAP) in the form of rudimentary soft tissue masses is quite common. Management involves ligation or surgical excision. Rarely do literature discussions cover complex variants in which the extra finger is fully developed.

The present study aims to demonstrate the radiological angular parameters of a sample of patients treated at our institution and to compare the radiological abnormalities with other classifications or parameters from the literature.  We evaluated a sample of patients submitted to panoramic radiographic examinations of the lower limbs. The inclusion criteria were: (1) Patients without knee osteoarthritis as assessed by an orthopedist.

Article Synopsis
  • This study examined the effectiveness of hemi-epiphysiodesis in treating adolescents with idiopathic genu varum and valgum, focusing on the impact of the distal femur and proximal tibia.
  • Results showed that in the varus group, the proximal tibia contributed significantly to the deformity, while in the valgus group, both the distal femur and the proximal tibia played roles, with distal femur being more prominent.
  • The findings highlight the need for tailored surgical planning based on the specific contributions of these bone areas to effectively address each condition.
