Purpose: Despite significant improvements in the design and performance of continuous flow left ventricular assist devices (CFLVADs), one of the most important reasons hampering further penetration of this technology is the occurrence of adverse events, especially strokes. One of the well-known risk factors for strokes is hypertension which is particularly common in patients undergoing a CFLVAD implant. While the device is implanted in the heart, strokes happen due to pathology in the brain and we hypothesised that modelling the blood flow in the circle of Willis might shed light on the causation of strokes in this situation.

Background And Purpose: While the pulsatility index (PI) measured by transcranial Doppler (TCD) has broader associations with outcomes in neurocritical care, its use in monitoring delayed cerebral infarction (DCI) in patients with aneurysmal subarachnoid hemorrhage (SAH) is not endorsed by current clinical guidelines. Recognizing that arterial pressure gradient (ΔP) can be estimated using PI, we investigated the potential significance of TCD-estimated ΔP.
Methods: In this observational study of 186 SAH patients, we recorded the mean cerebral blood flow velocity (mCBFV) and PI values from the middle cerebral artery, along with corresponding blood pressures.

The effect of acetazolamide on regional brain tissue oxygenation in patients with acute brain injury (ABI) is unknown. We studied adult patients with ABI who received acetazolamide as per the treating physician's decision and had ICP and brain oxygen pressure (PbtO) monitoring. Baseline measurements of ICP, cerebral perfusion pressure (CPP), and PbtO were taken before administering acetazolamide; subsequent measurements were recorded every 5 min for a total of 20 min.

Traumatic direct type carotid cavernous fistula (CCF) is an acquired arteriovenous shunt between the carotid artery and the cavernous sinus post severe craniofacial trauma or iatrogenic injury. We reported a 46-year-old woman who had developed a traumatic direct type CCF after severe head trauma with a skull base fracture and brain contusion hemorrhage. The clinical manifestations of the patient included pulsatile exophthalmos, proptosis, bruits, chemosis, and a decline in consciousness.

Calibre persistent artery of the lower lip is a vascular anomaly where the branches of the inferior labial artery maintain their size up to the submucosa of the lip. There is persistent pulsatile feeling, occasional ulceration, and recurrent bleeding. Doppler ultrasound and angiogram are used to confirm diagnosis.
