Purpose: Based on CARS Congress events selected from its 40 year history, this editorial summarises the main challenges and solution concepts encountered, and what the future may hold for a model-centric world view in the specific domain of computer assisted radiology and surgery.

Methods: Altogether some 15,000 publications appeared in the CAR/CARS Congress Proceedings and Journal between 1985 and 2025, comprising approximately 3000 full papers and 12,000 long abstracts. Modelling occupied a central theme in many of these publications, particularly in the 2020s.

Despite an increased understanding of the underlying biology of the disease, pancreatic ductal adenocarcinoma (PDAC) remains a challenging condition with poor outcomes and a 5-year survival rate, even at an early stage, of less than 10%. This dismal prognosis is due in part to biological aggressiveness, the lack of early diagnosis, suboptimal surgical selection, and the need for effective neoadjuvant and adjuvant therapies. However, progress is being made with significant advancements in the diagnosis, staging, and treatment of PDAC, with a focus on precision therapy, improved surgical care, and supportive care, coupled with a deeper understanding of PDAC's biology.

Background: Merkel cell carcinoma (MCC) is a rare but highly aggressive cutaneous malignancy. Immune checkpoint inhibition (ICI) with PD-(L)1 blockade has significantly improved treatment outcomes in metastatic disease. In patients with primary resistance to PD-(L)1 inhibition, a high overall response rate (ORR) of 50% to later-line ipilimumab plus nivolumab (IPI/NIVO) has been demonstrated.

Background: Among the Alzheimer's disease (AD) biomarkers measured in blood, phosphorylated forms of tau (p-tau) have been shown to exhibit a particularly high diagnostic potential. Here, we performed a comprehensive method comparison study, followed by evaluation of the diagnostic performance of eight recent plasma p-tau immunoassays targeting different tau phosphorylation sites, different tau fragments, and that are measured by two distinct platforms.

Methods: We enrolled a cohort of 40 patients with AD at the stage of dementia (AD-dem) characterized by positive CSF A + T + profile, and a control group of 40 cognitively healthy participants (Control), to conduct a comprehensive method comparison for three plasma p-tau181 and five plasma p-tau217 assays run on the Simoa HD-X™ or Lumipulse G600II/G1200 platforms.
