Optic nerve head (ONH) edema is a clinical manifestation of many ocular and systemic disorders. Ocular and central nervous system imaging has been used to differentiate the underlying cause of ONH edema and monitor the disease course. ONH vessel abnormalities are among the earliest signs of impaired axonal transportation. Optical coherence tomography angiography (OCTA) is a noninvasive method for imaging ONH and peripapillary vessels and has been used extensively for studying vascular changes in ONH disorders, including ONH edema. In this narrative review, we describe OCTA findings of the most common causes of ONH edema and its differential diagnoses including ONH drusen.

Methods: ONH and adult ONG patients were recruited from 2012 to 2022.
Results: A total of seven ONH patients (8 eyes) and 23 adult ONG patients (24 eyes) were assessed.

Purpose: In this study, we aimed to evaluate and compare the effects of intravitreal aflibercept (IVA) and intravitreal faricimab (IVF) injections on the blood flow of retinal vessels in the peripapillary region and optic nerve head (ONH) in eyes with diabetic macular edema (DME) using laser speckle flowgraphy (LSFG).
Patients And Methods: This study included 20 eyes of 18 patients treated with IVA and 15 eyes of 11 patients treated with IVF for DME. The mean blur rate (MBR) of the ONH and retinal artery and vein of the peripapillary region were measured using LSFG at baseline and 1 month after injection.

Purpose: To determine the characteristics of peripapillary hyperreflective ovoid mass-like structures (PHOMS) in patients with nonarteritic anterior ischemic optic neuropathy (NAION) and in normal adults.
Methods: A total of 406 included eyes were divided into four groups: acute NAION group, chronic NAION group, unaffected group, and normal eyes group. PHOMS were detected on optical coherence tomography slices from optical coherence tomography angiography scans centered on the optic nerve head (ONH).
Key Clinical Message: Infiltrative optic neuropathy in hemophagocytic lymphohistiocytosis is rare but could potentially lead to visual loss. Cytomegalovirus (CMV) optic neuritis, drug toxicity, and CNS involvement with increased intracranial pressure (ICP) are differential diagnoses that have to be considered.
Abstract: In this report, we introduced a known case of hemophagocytic lymphohistiocytosis (HLH) with progressive visual loss due to bilateral optic nerve head (ONH) involvement.
