Cytoreductive surgery and intraoperative, intraperitoneal hyperthermic chemoperfusion (hyperthermic intraperitoneal intraoperative chemotherapy) are commonly used for the management of peritoneal surface metastases. We describe a case of acute embolic infarcts in bilateral cerebellar lobes, cerebral peduncles, thalamus and left parietal lobe cortex in a postmenopausal woman in her 50s diagnosed with bilateral ovarian mucinous adenocarcinoma with peritoneal metastasis under general anaesthesia.

Although the precise pathophysiological mechanisms of UP remain elusive, the thalamus, which is integral to processing sensory information, is potentially implicated in its development. This case report discusses a 32-year-old woman with no significant medical history who underwent elective bilateral breast reduction with auto-augmentation and abdominoplasty. Initially, her recovery was smooth, but on the third postoperative day, she exhibited tachycardia, tachypnea, and mild shortness of breath, while other vital signs remained stable. A chest X-ray revealed pneumoperitoneum, and a subsequent abdominal CT confirmed moderate pneumoperitoneum without gastrointestinal leakage.

She was initially diagnosed eight years ago and remained disease-free for 52 months. However, she later experienced a recurrence, indicated by elevated inhibin B levels (58 ng/mL) and the presence of peritoneal soft tissue tumors.

Introduction: Mucinous appendiceal neoplasms are unique tumors in which >50 % of the tumor volume is composed of extracellular mucin. They may present as an unruptured mucin-filled appendix or, more commonly, with peritoneal metastases after rupture or transmural invasion of the primary tumor. This case report describes a case of presumed ovarian malignancy with final pathologic diagnosis of low grade appendiceal mucinous neoplasm.
