Implant-supported restorations have become a successful, predictable, and mostly a first-choice strategy for replacement of missing teeth. However, complications such as implant fracture, malpositioned or poorly angled implants, and peri-implantitis persist in implant dentistry. Management of these complications may necessitate removal of an osseointegrated implant. Traditionally, implant removal has been a surgically invasive procedure; however, minimally invasive explantation techniques have evolved substantially over the last few years. This digital presentation describes various scenarios where osseointegrated implants require explantation, decision-making considerations, and techniques for removal of osseointegrated implants.

Numerous studies have investigated the surface treatment of implants using various types of plasma, including atmospheric pressure plasma and vacuum plasma, to remove impurities and increase surface energy, thereby enhancing osseointegration. Most previous studies have focused on generating plasma directly on the implant surface by using the implant as an electrode for plasma discharge. However, plasmas generated under atmospheric and moderate vacuum conditions often have a limited plasma volume, meaning the shape of the electrodes significantly influences the local electric field characteristics, which in turn affects plasma behavior.

Objective: Removal of a transcutaneous osseintegrated endo-fix stem (ESKA Orthopaedic, Lübeck, Germany) following a fatigue fracture of the implant, whilst protecting the residual femur bone to allow transcutaneous osseointegrated prosthesis system (TOPS) reimplantation.
Indications: A patient's request for a further TOPS implantation following a fatigue fracture of a circular osseointegrated implant stem.
Contraindications: Impending destruction of the bone tube through mobilisation of the femoral implant stem with insufficient thickness of the cortical wall (< 2-3 mm).

The present study investigated the in vivo aging of yttria-stabilized zirconia (YSZ) oral implants (ZiUnite®) removed after 37 to 181 months. These implants featured a porous zirconia surface to enhance osseointegration. They were placed in prospective clinical investigations and had to be explanted due to peri‑implant bone breakdown.

Background: Implant fixation is often the cornerstone of musculoskeletal surgical procedures performed to provide bony fixation and/or fusion. The aim of this study was to evaluate how different design features and manufacturing methods influence implant osseointegration and mechanical properties associated with fixation in a standardized model in cancellous bone of adult sheep.
Methods: We evaluated the performance of three titanium alloy implants: (A) iFuse-TORQ implant; (B) Fenestrated Sacroiliac Device; and (C) Standard Cancellous Bone Screw in the cancellous bone of the distal femur and proximal tibia in 8 sheep.

Nonthermal plasma has been extensively utilized in various biomedical fields, including surface engineering of medical implants to enhance their biocompatibility and osseointegration. To ensure robustness and cost effectiveness for commercial viability, stable and effective plasma is required, which can be achieved by reducing gas pressure in a controlled volume. Here, we explored the impact of reduced gas pressure on plasma properties, surface characteristics of plasma-treated implants, and subsequent biological outcomes.
