Introduction: Although it is primarily a pulmonary disease, extra-pulmonary TB has been reported in 15 % of TB patients and it can affect any body system including the pleura. Pleural tuberculosis can result into empyema thoracis which can further complicate if left untreated to empyema necessitans. This requires high index of suspicious for prompt medical and surgical management.

Background: Most cases of chronic empyema are caused by acute thoracic empyema or tuberculous pleuritis. Open thoracotomy and decortication are traditional treatments for chronic empyema. However, some cases, such as those with thick calcifications around a large cavity, may be difficult to decorticate in a single surgery.

Empyema is defined as an accumulation of frank pus within the pleural cavity. Empyema necessitans is a rare complication of the pleural space infection when the existing empyema extends into adjacent structures beyond the pleural space, usually into the soft tissue of the chest wall. If severe, it can externalize to the skin.

Diagnosis of tuberculosis (TB) in pregnancy may be challenging. Recommended diagnostic tests often are not sensitive, and additional diagnostic procedures are necessary to confirm disease. Symptoms of TB in pregnancy are often atypical and difficult to diagnose in the early stages of the disease.
