AI Article Synopsis

  • The study explored the changes in coagulation and platelet functions during the perioperative period of transcatheter aortic valve implantation (TAVI) using thromboelastography (TEG) platelet mapping.
  • It involved 25 patients with measurements taken at three different time points: before heparinization, and on postoperative days one and three.
  • Results showed that while clot strength (MA) decreased until POD 3, coagulation capacity increased, indicating an antithrombotic change, and platelet function was notably affected by the ADP receptor system over the three-day period post-TAVI.

Article Abstract

Introduction: The possibility of hypercoagulability during the perioperative period of transcatheter aortic valve implantation (TAVI) has been noted; however, there is still a controversy regarding the appropriate perioperative antithrombotic therapy. The study investigated coagulation and platelet functions during the TAVI perioperative period using thromboelastography (TEG) 6s platelet mapping.

Methods: A prospective observational study was conducted on 25 patients undergoing TAVI. TEG platelet mapping was performed at three time points: on admission to the operating room (before heparinisation), on postoperative day (POD) 1 and on POD 3. Perioperative changes observed included: maximum clot strength (MA), clot strength without platelet function (MA), time to initiation of clots formation by coagulation factors (R) and platelet function (G). G is activated by thrombin, and not affected by antiplatelet agents. It is calculated as [(5000×MA)/(100 - MA)] - [(5000×MA)/(100 - MA)]. Finally, MA and G, which reflect clot strength and platelet aggregation mediated by ADP/thromboxane A receptors, respectively, were also examined using the same method as for G.

Results: MA continued to decrease until POD 3, indicating antithrombotic change after TAVI. G continuously decreased for 3 days after TAVI, while MA increased significantly on POD 3. Furthermore, R shortened on POD 1 and POD 3, suggesting increased coagulation capacity after TAVI. Finally, G in clopidogrel-naive patients was reduced for 3 days after TAVI, while G in aspirin-naive patients showed no significant change perioperatively.

Conclusions: In this study involving TEG platelet mapping, coagulation capacity increased while platelet function decreased, resulting in antithrombotic change for 3 days after TAVI. The ADP receptor system may be implicated in the decreased platelet function. These results may be useful for considering optimal perioperative antithrombotic therapy in TAVI.

The effect of physical cues on platelet storage lesion.

Background: Platelet concentrates play an important role in clinical treatment such as platelet function disorders and thrombocytopenia. In the process of preparation and storage of platelets, centrifugation, leukofiltration, and agitation will cause morphological changes and impaired function of platelets, which is associated with the increase of platelet transfusion refractoriness, and named as platelet storage lesion (PSL).

Method: This paper proposes three major operations (centrifugation, agitation, and leukofiltration) that platelets experience during the preparation and storage process, to explore the effect of physical cues on PSL.

Background: Reduced effect of antiplatelet therapy has been reported in patients with ST-segment elevation myocardial infarction (STEMI). This could partly be explained by an increase of highly reactive immature platelets.

Objectives: To investigate changes in platelet maturity and reactivity after acute STEMI.

Thromboxane A (TXA), a prothrombotic factor that induces platelet aggregation and thrombosis, acts as a vasoconstrictor by activating TXA receptors (TP receptors). TXA is extremely unstable and metabolizes into three major metabolites: 2,3-dinor thromboxane B (2,3-dinor-TXB), 11-dehydro TXB(11-dh-TXB), and 11-dehydro-2,3-dinor TXB(11-dh-2,3-dinor-TXB). 8-Iso-prostaglandin F(8-iso-PGF), a prostaglandin-like compound widely considered the best biomarker of oxidative stress, can also activate TP receptors.

Background: Platelet count and function may be closely related to survival and prognosis of stroke and cancer. However, little is known on the impact of platelet count on the patients with a history of stroke and cancer. This study aimed to examine the association between baseline platelet level and all-cause mortality in this population using a cross-sectional analysis.

Background: Hypertension has shown a trend of prevalence at younger ages, and the non-dipping pattern is associated with target organ damage in hypertension. However, few studies have yet investigated the clinical characteristics and risk factors of non-dipper status in essential hypertension children. This study aimed to explore the clinical characteristics and possible indicators associated with non-dipper status in children with essential hypertension.
