Introduction: A bronchopleural fistula (BPF) after an anatomical lung resection commonly arises singly. We report a case of a metachronous BPF, which developed after omentoplasty of a preceding fistula and subsequently closed without any intervention.

Case Presentation: A 77-year-old patient underwent omentoplasty for a brochopleural fistula (BPF) following a right lower lobectomy. A sudden massive air leak developed from the novel BPF approximately 1 cm proximal to the preceding fistula 3 days later. The air leak resolved spontaneously without any intervention one week later. The corresponding fistula was found to be completely closed. Computed tomography showed the omental flap covered both fistulae.

Conclusion: The present case suggested that a metachronous BPF could develop and a harvested omental flap might migrate even after being anchored.

Medical Gaze Sans Bioethics: Revisiting Enslaved Black Women's Medical Bondage in Behind the Sheet.

The birth of modern gynecology in the USA is preceded by experimental exploitations of Black women's bodies in the mid-nineteenth century, entailing a long-drawn extraction of "reproductive knowledge" from enslaved patients. Charly Evon Simpson's Behind the Sheet (2019) stages the history of medical bondage of Black enslaved women in antebellum South, reconstructing the events that led to the surgical innovation for vesico-vaginal fistula. Scrutinizing Simpson's dramatization of the event, this paper prompts inquiries into the interplay of power and consent between the physician and the enslaved patient in plantation healthcare, highlighting the need to reexamine bioethical principles.

Article Synopsis
  • Venous aneurysms associated with dural arteriovenous fistula (dAVF) typically arise from long-standing venous hypertension and present as an enlargement of the draining vein, but some saccular aneurysms can occur without clear evidence of hypertension, making them quite rare.
  • This report highlights two cases of ruptured saccular venous aneurysms linked to dAVF, both lacking signs of venous hypertension and exhibiting significant curvature or narrowing in the draining veins, which were successfully treated using transarterial embolization.
  • The findings suggest that while hemodynamic stress is important in the formation of venous aneurysms, prior venous hypertension is not essential for their development, emphasizing the

Endoscopic or combined management of post-surgical biliary leaks: a two-center recent experience.

Surg Endosc

December 2024

Endoscopy Service, Department of Diagnostic and Therapeutic Services, IRCCS - ISMETT, Palermo, Italy.

Background And Aims: Post-surgical biliary leaks (PSBL) are one of the most prevalent and significant adverse events emerging after liver or biliary tract surgeries. Endoscopic retrograde cholangiopancreatography (ERCP) alone or combined with another approach (Rendez Vous) as treatment of PSBL obtains optimal outcomes due to the possibility of modifying the resistances in the biliary tree.

Methods: A retrospective double-center study was conducted in two tertiary centers.

Rare diseases: a challenge in paediatric dentistry.

Article Synopsis
  • - Rare diseases, though infrequent (happening in 100-500 per million), pose significant challenges in pediatric dentistry due to their complex needs and the required specialized care.
  • - Conditions like X-linked hypophosphatemic rickets (XLH), hypophosphatasia (HPP), and osteogenesis imperfecta (OI) show how systemic health problems can directly affect oral health, necessitating a multidisciplinary approach for effective treatment.
  • - XLH is particularly notable, caused by mutations in the PHEX gene, leading to issues such as poor enamel mineralization, dental abscesses, and unique dental manifestations, emphasizing the crucial role dentists play in early diagnosis and referrals to genetic specialists.
