Background: Primary central nervous system lymphomas (PCNSLs) are sensitive to chemotherapy. The standard treatment is high-dose methotrexate (MTX)-based chemotherapy. There are no reports of successful treatment of acute uric acid nephropathy with rasburicase after MTX administration in PCNSLs.

Case Presentation: A 54-year-old man with a history of gout presented with a change in character and cognitive dysfunction. MRI showed a large enhancing mass spanning the bilateral frontal lobes and the right temporal lobe. After endoscopic biopsy, an MTX, procarbazine, and vincristine (MPV) regimen was initiated for the treatment of the PCNSL. After the initiation of chemotherapy, the patient experienced a gout attack, and blood examination revealed acute renal failure (ARF) and hyperuricemia. The considered causes of ARF included MTX toxicity and acute uric acid nephropathy. As the dramatic effect of MTX was observed, treatment was continued despite ARF, most probably due to acute hyperuricemia due to tumor lysis, which was treated in parallel. After an improvement in renal function, MTX was resumed, and rasburicase was initiated to control hyperuricemia. A complete response was obtained after induction chemotherapy. Hyperuricemia was controlled with rasburicase, and renal function was preserved.

Conclusions: Acute uric acid nephropathy should be considered when ARF occurs after the initiation of MTX in PCNSLs, especially in newly diagnosed PCNSL patients with large tumors or hyperuricemia.

Article Synopsis
  • Nephrotoxicity is a major issue with tyrosine kinase inhibitors like dasatinib (DASA), which can cause renal tubular cell damage.
  • Naringenin (NGN) has antioxidant and anti-inflammatory effects and may provide protection against DASA-induced acute kidney injury, as shown in a study where mice pre-treated with NGN had improved kidney function and lower levels of harmful substances after DASA administration.
  • NGN was found to reduce oxidative stress, restore antioxidant levels, and decrease inflammation markers, suggesting it may protect kidneys by counteracting the negative effects of DASA.

Article Synopsis
  • This study examines the role of serum uric acid (SUA) levels in patients undergoing off-pump coronary artery bypass (OCABG) surgery, focusing on their impact on postoperative complications like acute kidney injury (AKI).
  • The research involved 144 patients and found that 20% experienced AKI, with higher preoperative SUA levels significantly linked to increased risk (OR: 2.04).
  • Additionally, elevated SUA levels correlated with longer hospital stays and a higher mortality rate (up to 13% in high SUA patients), indicating that SUA is a crucial predictor for postoperative outcomes.

A case of transient elevation of creatinine caused by severe hyperuricemia.

Clin Chim Acta

December 2024

Departments of Laboratory Medicine, Peking Union Medical College Hospital, Chinese Academy of Medical Sciences, Beijing 100730, China. Electronic address:

Background: To explore the underlying causes of significant fluctuations in creatinine levels within three days due to transient and severe uric acid elevation, and to provide evidence for the interpretation of abnormal test results and clinical diagnosis and management.

Methods: The issues were resolved by retesting the samples, comparing results across different detection platforms, and analyzing the reaction curve. We comprehensively reviewed patients' general conditions, imaging examinations, and treatments.

: Clinical findings have shown a negative correlation between the severity of depressive symptoms and serum uric acid levels in men, yet the role of metabolic regulation in the pathophysiology of depression remains largely unknown. : In this study, we utilized an acute restraint-stress-induced male rat model of depression to investigate biochemical changes through NMR-based metabolomics combined with serum biochemical analysis. Additionally, we employed qPCR, immunoblotting, and enzyme activity assays to assess the expression and activity of xanthine oxidoreductase, the rate-limiting enzyme in uric acid production.

Background: Uric acid (UA) plays an important role in cardiovascular diseases, yet its implications in elderly patients remains incompletely understood. This study aimed to explore the impact of UA on the prognosis in advanced-age patients with acute coronary syndrome (ACS).

Methods: We included 526 patients aged 80 and older who were diagnosed with ACS.
