A 48-year-old man had a 3-year history of progressive sensory and motoric disturbance of the lower extremities. Computed tomography and magnetic resonance imaging revealed an extensive epidural lipoma reaching from T2 to T10 level. After hemilaminectomy the dorsally located epidural fat was removed. Ten months later the neurological deficit had greatly improved.
