An Unusual Case of Postpartum Empyema.

We present the case of a 32-year-old woman with a left empyema and T12 osteomyelitis resulting from group B Streptococcus infection occurring 3 weeks after instrumental delivery of a healthy boy. Empyema is a rare complication of instrumental delivery, and this patient highlights the maternal risk resulting from group B Streptococcus bacteremia.

Introduction: Group B Streptococcus (GBS) is an opportunistic pathogen that can induce chorioamnionitis (CA), increasing the risk of neurodevelopmental disorders (NDDs) in the offspring. The placenta facilitates maternal-fetal communication through the release of extracellular vesicles (EVs), which may carry inflammatory molecules such as interleukin (IL)-1. Although the role of EVs in immune modulation is well established, their specific characterization in the context of GBS-induced CA has not yet been investigated.

Background: Pneumococcal conjugate vaccines (PCVs) introduced in childhood national immunization programs lowered vaccine-type invasive pneumococcal disease (IPD), but replacement with non-vaccine-types persisted throughout the PCV10/13 follow-up period.
