Engaging and motivating busy trainees to work on reducing the climate impact of their clinical practice is challenging. To our knowledge, there are no published studies of graduate medical education (GME)-wide, institutional efforts to engage residents in implementing climate sustainability improvement projects. We piloted a novel, institution-wide, pay-for-performance (P4P) sustainability quality improvement (SusQI) program in 2023-2024 that enabled residents from all GME programs to implement SusQI projects with practice-changing improvement goals for a financial incentive.

Aims: To compare the prevalence of GDM and pregnancy outcomes between the one-step and two-step methods of universal screening among Thai pregnant women.
Methods: A randomized controlled trial was conducted on singleton Thai pregnant women at a gestational age of 24-28 weeks. They were randomly assigned to either the one-step method group (a universal 75-gm 2-h oral glucose tolerance test: OGTT) or the two-step method group (a universal 50-gm oral glucose challenge test followed by a 100-gm 3-h OGTT).

Animal law has the potential to initiate improvements for animal wellbeing. However, this largely depends on how effectively the law bridges the legal chasm between animal welfare and animal suffering, a chasm the authors refer to as the welfare gap. When the law does not adequately address this gap, where regulation subordinates animal interests to human interests, it results in weak animal protection that does little more than regulate to a standard that avoids a life not worth living.
