To clinically and radiographically evaluate patient-specific titanium meshes via a trans-antral approach for correction of enophthalmos and orbital volume in patients with recent unilateral orbital floor fracture. Seven patients with unilateral orbital floor fractures received patient-specific titanium meshes that were designed based on a mirror-image of the contralateral intact orbit. The patient-specific implants (PSIs) were inserted via a trans-antral approach without endoscopic assistance.

Choanal atresia is one of the more commonly observed congenital abnormalities of the nose. The condition is predominantly found in females with a M:F ratio of 1:2. 65-75 % of patients with choanal atresia are unilateral, and the rest are bilateral.

Surgical decompression of the orbit may be necessary in dysthyroid patients with compressive optic neuropathy. Two-wall decompression with a trans-antral or a trans-conjunctival approach is commonly used. However, in some patients the initial improvement following this surgical procedure is not maintained.

A technique of antro-ethmosphenoidectomy has evolved over the last 50 years to provide a thorough, yet safe method of treating chronic, recurrent advanced pansinusitis. The trans-antral approach to the ethmoid labyrinth and sphenoid sinuses was first described before the turn of this century. A brief chronological history is reviewed in order to understand the development and then the abandonment of this surgery for over twenty years.
