Background: Gastrodia elata Blume (GEB), a traditional Chinese medicine, has been widely used to treat dizziness, numbness of limbs, and infantile convulsion, among other issues. Gastrodin is the main component of GEB. This meta-analysis aimed to evaluate the efficacy and safety of gastrodin in the treatment of migraine.

Methods: Ten electronic databases, namely the Cochrane Library, Embase, EBSCO, PubMed, Web of Science, CENTRAL, CNKI (China National Knowledge Infrastructure), CBM (Chinese Biomedicine Database), WanFang, and VIP (Chinese Scientific Journals Database), were searched for randomized controlled trials (RCTs) of gastrodin for migraine published before September 2021. The data were analyzed by RevMan 5.3 software and evaluated by GRADEpro.

Results: A total of 1,332 subjects were included in 16 RCTs. The meta-analysis showed that gastrodin was significantly effective in treating migraine (RR = 1.21, 95%CI = [1.17, 1.27]), reducing the pain degree (MD = -1.65, 95% CI = [-2.28, -1.02]), reducing the frequency of migraine attack (SMD = -2.77, 95% CI = [-3.92, -1.62]), shortening the duration of migraine attack (SMD = -1.64, 95% CI = [-2.35, -0.93]), and slowing average arterial cerebral blood flow velocity (SMD = -3.19, 95% CI = [-5.21, -1.17]), as well as being safe.

Conclusions: This systematic review revealed gastrodin is effective and safe in the treatment of migraine.

Article Synopsis
  • Tianshu capsule (TSC) is a traditional Chinese medicine that shows promise in treating migraines, prompting a study to explore its compound effects and pharmacokinetics.
  • The research utilized advanced chromatography techniques to identify 46 compounds in blood and brain of rats after TSC administration, highlighting four key ingredients (Gastrodin, ferulic acid, senkyunolide G, and senkyunolide I).
  • Findings revealed that these compounds, especially senkyunolide I, quickly reached the brain and showed differences in absorption between healthy and migraine-affected rats, indicating a need for deeper investigation into TSC's therapeutic mechanisms.

Ethnopharmacological Relevance: Ischemic stroke (IS) a complex pathological event emerging as one of the most serious threats with huge economic impact in the 21st century. Following IS, multiple cascades and pathways are stimulated, culminating in long term consequences. One of Chinese Traditional Medicine, Tianma Siwu Decoction (TSD), is known to have sedative-hypnotic, anticonvulsant and anti-inflammatory effects, which is usually used to treat migraine and ischemic stroke, but its potential pharmacological mechanism remains unclear.

Ethnopharmacological Relevance: Quality control is a critical element for Traditional Chinese medicine (TCM). Due to the varied chemical components, mechanisms of action, and pharmacological functions in TCM, ensuring quality is more challenging compared to chemical drugs. Then, the concept of quality markers (Q-markers) was proposed and ideal Q-markers for TCM prescriptions need to compliant with "five principles", including pharmacological effectiveness, specificity, transfer and traceability, measurability, and prescription compatibility.

Background: The quality of traditional Chinese medicine (TCM) is the prerequisite for ensuring its safe and effective clinical application. With the increasing popularity of TCM worldwide, the quality control of TCM products has become increasingly crucial. Tianma toutong tablet (TMTTT) is mainly used for migraine caused by external wind and cold, blood stasis, or deficiency of blood and nourishment.

Background: Gastrodin (GAS), a main bioactive component of the herbal plant, Gastrodia elata Blume, has shown to have beneficial effects on neuroinflammatory diseases such as Alzheimer's disease in animal studies and migraine in clinical studies. Inflammasome is a multimeric protein complex having a core of pattern recognition receptor and has been implicated in the development of neuroinflammatory diseases. Gastrodin has shown to modulate the activation of nucleotide-binding oligomerization domain (NOD)-like receptor protein 3 (NLRP3) inflammasome.
