Article Synopsis
  • The study aimed to investigate how different patterns of blood flow in the kidneys, assessed through Doppler ultrasound, can indicate the severity of venous congestion and predict kidney function impairment in patients suffering from acute decompensated chronic heart failure (ADCHF).
  • Conducted on 75 ICU patients, the study analyzed the blood flow patterns (normal, biphasic, monophasic) and measured sodium concentration in urine after diuretic treatment to monitor kidney injury and other complications.
  • Results revealed that patients with a monophasic blood flow pattern had a significantly higher incidence of acute kidney injury (100%) and were more likely to experience in-hospital death and diuretic resistance compared to those with normal or biphasic patterns

Due to the potential benefits of triamterene in diuretic resistance, this study was performed to assess whether triamterene add-on to the standard treatment of heart failure (HF)-related diuretic resistance improves outcomes. A randomized clinical trial was performed on 45 hospitalized patients with HF with reduced ejection fraction who had diuretic resistance. Patients were randomized to receive either triamterene 50 mg plus hydrochlorothiazide 25 mg (n = 23) or hydrochlorothiazide 50 mg alone (n = 22) until hospital discharge.

Activated proteinC resistance is a frequent prothrombotic abnormality. In most cases it is due to factorV Leiden mutation by nucleotide G1691A substitution. This recently described thrombophilic defect of activated proteinC resistance has been postulated to be implicated in the pathogenesis of idiopathic intracranial hypertension (IIH).

The term acute heart failure (AHF) refers to a clinical syndrome with typical symptoms and signs, in which a structural or functional heart abnormality leads to defective oxygen delivery. The term cardiorenal syndrome has been proposed to outline the strict interplay between cardiac and renal function. In the setting of acute cardiac decompensation, acute kidney injury (AKI) is generally referred to as cardiorenal syndrome type 1.
