An 86-year-old female was taken to hospital with complaints of general malaise and anorexia. Echocardiography showed an abnormal space between the ventricles, extending to the back of the left atrium, with a shunt from the left ventricle into both that abnormal space and the right ventricle. The next morning, the patient had a large amount of tarry stool and progressive anemia.

The clinical manifestation typically involves pronounced hemodynamic compromise necessitating prompt surgical intervention. A 59-year-old man was admitted to our hospital with left hemiplegia. A computed tomography( CT) scan and echocardiography revealed a cerebral infarction in the right middle cerebral artery's territory, as well as a large pseudoaneurysm (4×3 cm) of the lateral left ventricular wall. The patient agreed to undergo cardiac surgery because of the high risk of rupture and recurrent cerebral infarctions.

Aortopulmonary window (APW) is a rare anomaly with variable morphology and associated cardiac anomalies. We evaluated impact of patient and operative factors on mid-term outcomes following APW repair. Twenty-nine patients underwent surgical APW repair at our institution from 1996 to 2022.
