Colonic Acquired Fistulae: The Potential of CT Colonography.

The diagnosis of colonic fistulae is crucial in defining the best therapeutic approach. Fistulae can be suspected from clinical history or specific symptoms, but imaging plays a key role in confirming the diagnosis. Previous studies described conventional studies with barium as methods of choice for the diagnosis; more recently intestinal ultrasound, CT and MR have been considered promising tests. CT Colonography is the best radiologic test to evaluate the colon, but only few case reports described its role in this setting. Aim of this paper is to highlight the potentials of CT colonography in evaluating patients with suspected colonic fistulae.

We present the case of an 18-year-old male who was diagnosed at age 15 with extensive ileal and colonic Crohn's disease with rectal involvement and associated perianal fistula. The patient received different treatments with biologics withouth achieving response, so we decide to try combination therapy with subcutaneous Infliximab and Risakizumab. The patient showed improvement at second week, achieving both analytical, clinical and endoscopic response .

We report a rare case of nephrocolic fistula in a 56-year-old female presenting with a three-month history of altered bowel habits. Initial colonoscopy revealed a sinus tract in the descending colon. Her hemoglobin was 79 g/L, and she had a history of nephrolithiasis and trauma.
