Amyloid transthyretin cardiomyopathy is a progressive disease that confers significant mortality. While it is relatively rare, the frequency of diagnoses has risen with the increased contribution of novel diagnostic approach over the last decade. Traditionally tissue biopsy was considered to be a gold standard for amyloidosis diagnosis. However, there are significant limitations in the wide application of this approach. A noninvasive imaging-based diagnostic algorithm has been substantially developed in recent years. Establishing radionuclide imaging standards may translate into a further enhancement of disease detection and improving prognosis in the group of patients. Therefore we present in the following document current evidence on the scintigraphic diagnosis of cardiac transthyretin amyloidosis. Moreover, we present standardized protocol for the acquisition and interpretation criteria in the scintigraphic evaluation of cardiac amyloidosis.

Introduction: Advancements in scintigraphic gastrointestinal motility testing that were developed, validated, and replicated in clinical practice at Mayo Clinic have significantly enhanced our understanding and management of gastrointestinal motility disorders. Importantly, normative values in at least 100 healthy controls and estimated inter- and intra-individual coefficient of variation for all parameters have been published for these tests.
Areas Covered: This article reviews methodology, accuracy, reproducibility, and diagnostic cutoffs of scintigraphic techniques for measuring gastric emptying, gastric accommodation, and colonic transit, comparing methods from Mayo Clinic with those used elsewhere when appropriate.

Purpose: While early diagnosis is crucial, as new treatments can significantly slow the progression of the disease, there is growing evidence on the application of novel imaging techniques for detecting transthyretin amyloidosis (ATTR) in pre-symptomatic stages. This study aimed to evaluate the utility of pre-symptomatic scintigraphic imaging cascade screening for early detection of ATTR.
Methods: During the period from 2020 to 2024, we conducted a prospective study that enrolled 100 consecutive adults.
