Purpose: Complexity in selecting optimal non-coplanar beam setups and prolonged delivery times may hamper the use of non-coplanar treatments for nasopharyngeal carcinoma (NPC). Automated multi-criterial planning with integrated beam angle optimization was used to define non-coplanar VMAT class solutions (CSs), each consisting of a coplanar arc and additional 1 or 2 fixed, non-coplanar partial arcs.

Methods: Automated planning was used to generate a coplanar VMAT plan with 5 complementary computer-optimized non-coplanar IMRT beams (VMAT+5) for each of the 20 included patients. Subsequently, the frequency distribution of the 100 patient-specific non-coplanar IMRT beam directions was used to select non-coplanar arcs for supplementing coplanar VMAT. A second investigated CS with only one non-coplanar arc consisted of coplanar VMAT plus a partial arc at 90° couch angle (VMATCS90). Plans generated with the two VMATCSs were compared to coplanar VMAT.

Results: VMAT+5 analysis resulted in VMATCS60: two partial non-coplanar arcs at couch angles 60° and -60° to complement coplanar VMAT. Compared to coplanar VMAT, the non-coplanar VMATCS60 and VMATCS90 yielded substantial average dose reductions in OARs associated with xerostomia and dysphagia, i.e., parotids, submandibular glands, oral cavity and swallowing muscles (p < 0.05) for the same PTV coverage and without violating hard constraints. Impact of non-coplanar treatment and superiority of either VMACS60 and VMATCS90 was highly patient dependent.

Conclusions: Compared to coplanar VMAT, dose to OARs was substantially reduced with a CS with one or two non-coplanar arcs. Preferences for coplanar or one or two additional arcs are highly patient-specific, balancing plan quality and treatment time.

Background: This study aimed to investigate the effect of couch rotation angles on non-coplanar volumetric modulated arc therapy (ncVMAT) plan for stereotactic body radiotherapy (SBRT) in lung cancer patients and to evaluate the feasibility of clinically applying ncVMAT for SBRT.

Methods: Twenty-four lung cancer patients with a single lesion eligible for SBRT were enrolled in the study. Seven dual partial-arc VMAT plans with varying couch angles were designed for every patient.

Article Synopsis
  • This study explored the effectiveness and safety of three advanced stereotactic body radiation therapy (SBRT) techniques for treating pancreatic cancer: CyberKnife (CK), Tomography Radixact (TOMO), and Halcyon volume rotation intensity modulation therapy (VMAT).
  • Sixteen patients were analyzed using different SBRT plans, with a focus on optimization methods and median radiation doses.
  • Results indicated that CK with multi-leaf collimator (CK-MLC) provided the best protection for surrounding organs while effectively targeting the tumor, demonstrating its superior dosimetric properties compared to the other methods.

Background: Non-coplanarity and mixed beam modality could be combined to further enhance dosimetric treatment plan quality. We introduce dynamic mixed beam arc therapy (DYMBARC) as an innovative technique that combines non-coplanar photon and electron arcs, dynamic gantry and collimator rotations, and intensity modulation with photon multileaf collimator (MLC). However, finding favorable beam directions for DYMBARC is challenging due to the large solution space, machine component constraints, and optimization parameters, posing a highly non-convex optimization problem.

Background: Brain metastases are the most common intracranial malignancy and remain a substantial source of morbidity and mortality in cancer patients. Linear accelerator based stereotactic radiosurgery (SRS) is widely used and is frequently delivered by hypo-fractionnated volumetric modulated arc therapy using non-coplanar beams, where geometric accuracy and planning margins are a major concern.

Purpose: To give a practical analysis of intrafraction patient motion for multi-target, single isocentre, brain SRS treatments and to derive adapted GTV-to-PTV margins.

Introduction Esophageal cancer remains a leading cause of cancer-related mortality worldwide, with chemoradiotherapy being a cornerstone of its treatment. Ensuring precise radiation delivery is critical, as it minimizes exposure to surrounding healthy tissues, particularly vital structures like the heart and the left anterior descending artery (LAD). Volumetric arc therapy (VMAT) and intensity-modulated radiotherapy (IMRT) are two advanced radiotherapy techniques that offer enhanced dose conformity and reduced toxicity.
