Sudden-onset squint.

Dev Med Child Neurol

Published: April 1987

Differentiating a sudden-onset concomitant squint from a paralytic squint in an ill child can be difficult, but the distinction is important in view of the neurological implications of the latter. Furthermore, the two types may co-exist, and four such cases are described. The causes of sudden-onset squint are discussed. Methods of examination are described which may help to distinguish a paretic squint, with its neurological implications, from a concomitant deviation with no such systemic associations.

Acute acquired comitant esotropia (AACE) is a concomitant strabismus characterized by sudden onset, mostly associated with diplopia. The prevalence of AACE has significantly increased, and various management approaches have been recommended in recent years. This study by the Council of Asia-Pacific Strabismus and Pediatric Ophthalmology Society aimed to provide an overview of the clinical features, etiology and the nonsurgical and surgical treatment recommendations for the condition to equip strabismus specialists with the most updated knowledge.

The authors report a case of a 66-year-old man with sudden-onset diplopia and redness in the left eye. The examination revealed left hypotropia with exotropia and limited elevation. Contrast-enhanced computed tomography and histopathology suggested inferior rectus myositis with fibrosis.

Article Synopsis
  • The study investigates the use of botulinum toxin A (BTX-A) in treating acute acquired comitant esotropia (AACE) in children and young adults, particularly related to increased screen time during the COVID-19 pandemic.
  • Researchers analyzed data from 27 patients who received BTX-A injections to correct misalignment of the eyes caused by excessive use of devices like laptops and smartphones, focusing on the changes in eye deviation and binocular vision post-treatment.
  • Results indicated that BTX-A was effective, with significant improvements in eye alignment and visual function noted, especially when treatment was administered early after symptom onset.

Advances in the diagnosis and treatment of acute acquired comitant esotropia.

Acute acquired comitant esotropia (AACE) is mainly characterized by sudden onset, accompanied by diplopia, without extraocular muscles paralysis or ocular motility disorders. In recent years, the incidence of AACE has been increasing, researchers have found that this phenomenon may be related to the widespread use of electronic devices and the increase in the number of people working from home during the COVID-19 pandemic. However, its neural mechanisms have not been fully elucidated.
