We describe 3 women in 1 family who had systemic urothelial leukoplakia with no underlying pathological condition of the urinary tract. Irritative clinical symptoms resolved, and the macroscopic and microscopic appearance improved after 6 months of therapy with sodium pentosanpolysulfate. Electron microscopy showed special cells in the basal layer of the cornified squamous epithelium that also have been found in the trigone of women with so-called squamous metaplasia. Their similarity to Merkel's cells of the skin supports the theory that ectodermal cells are misplaced during embryogenesis. Genetic factors and sex hormones seem to influence the development of certain variants of squamous epithelium within the urinary tract.

A characteristic feature of fenretinide is the ability to inhibit cell growth through the induction of apoptosis with mechanisms that may be both receptor-dependent and receptor-independent. Chemopreventive efficacy of fenretinide has been investigated in clinical trials targeted at different organs. Results of a phase III secondary prevention trial suggest a benefit in preventing second breast malignancies in premenopausal women with early breast cancer.

A morphological analysis of 346 biopsy specimens from 140 patients with chronic cystitis permits one to evaluate a nonproliferative and proliferative forms of the disease as well as the squamous-cell metaplasia (leukoplakia) of urothelium as the risk diseases in the development of bladder cancer. Among them only the cases with dysplasia foci of urothelium or the squamous-cell metaplasia with acanthosis or dysplasia from the risk group belonging to bladder precancer. The early ultrastructural signs of urothelial dysplasia are determined; among them the proliferation of the undifferentiated cells with an increased nuclear-cytoplasmic ratio, the disappearance of asymmetrical mutual membrane, vesicular apparatus and cytoplasmic polarity of the superficial cells are to be distinguished.

The records of 108 patients presenting with leukoplakia of the urinary tract during the last 35 years were reviewed to define the natural history of this disease, with emphasis upon its association with urothelial cancer. Of the 108 patients 24 had upper urinary tract, 78 bladder and 10 urethral leukoplakia (1 with renal and bladder involvement, 3 with bladder and urethral involvement, and 1 with ureteral and renal involvement). Presenting complaints were primarily irritative.
