Published: June 2022

A 13-year male patient with a history of tachycardia attacks was diagnosed to have left posterior fascicular ventricular tachycardia (VT) according to the electrocardiogram (ECG) obtained at the emergency service. This diagnosis was confirmed with advanced electrophysiological studies and the case was diagnosed by genetic evaluation, which was performed to reveal the underlying cause, to have Brugada type 2 syndrome that might be associated with sudden cardiac death. Underlying causes should be evaluated, although idiopathic VTs generally have a good prognosis. Key Words: Brugada syndrome, Posterior fascicular ventricular tachycardia, Right bundle branch block.

Complex ventricular tachycardias involving the fascicular system (fascicular ventricular tachycardias [FVTs]) can be challenging. In this review, we describe our approach to the diagnosis and ablation of these arrhythmias with 10 illustrative cases that involve (1) differentiation from supraventricular tachycardia; (2) assessment for atypical bundle branch reentry and other interfascicular FVTs; (3) examination of P1/P2 activation sequences in sinus rhythm, pacing, and tachycardia; and (4) entrainment techniques to establish the tachycardia mechanism and aid circuit localization. To summarize, 5 cases had prior ablation with 2 previously misdiagnosed as supraventricular tachycardia.

Article Synopsis
  • Left bundle branch pacing (LBBP) is a newer technique for patients with heart failure, but its effectiveness varies depending on where the pacing is applied.
  • The study involved 43 patients who were divided into three groups based on their pacing sites, and results showed that all groups effectively narrowed QRS complexes and improved heart function.
  • LBTP (left bundle branch trunk pacing) had the best outcomes, especially in mechanical synchrony and overall heart efficiency, indicating that the choice of pacing site matters for treatment success.

Background: Variation in human left bundle branch (LBB) anatomy has a significant effect on the sequence of left ventricular depolarization. However, little is known regarding the electrophysiological characteristics of pacing different LBB fascicles.

Objective: We aimed to analyse the different electrocardiographic characteristics of LBB pacing (LBBP) attending to the site of pacing at the LBB system.
