Very-low-carbohydrate diets (LCHF; <50g/day) have been debated for their potential to lower pre-exercise muscle and liver glycogen stores and metabolic efficiency, risking premature fatigue. It is also hypothesized that carbohydrate ingestion during prolonged exercise delays fatigue by increasing carbohydrate oxidation, thereby sparing muscle glycogen. We report the case of a patient with type 2 diabetes mellitus (T2DM) on insulin therapy with a history of recurrent and severe hypoglycemia related to lipodystrophy with an uncommon clinical presentation. This was the case of a 67-year-old female with type 2 diabetes hospitalized for the exploration and management of severe and recurrent hypoglycemia. Introduction: Laron syndrome (LS) is a rare autosomal recessive disorder caused by mutations in the growth hormone (GH) receptor gene, resulting in GH resistance and reduced levels of insulin-like growth factor 1 (IGF-1). Patients with LS exhibit severe growth retardation, low IGF-1 levels, elevated basal GH, and poor response to GH stimulation.
