Background: Sickle cell disease, a debilitating, inherited haemolytic anaemia with premature morbidity and mortality, affects millions globally. Mitapivat, a first-in-class, oral, allosteric activator of pyruvate kinase, improves red blood cell survival by increasing ATP and diminishes sickling by decreasing 2,3-diphosphoglycerate. We aimed to evaluate the efficacy and safety of mitapivat in patients with sickle cell disease.

Outcome Factors for Women Undergoing Transcatheter Aortic Valve Replacement.

Interv Cardiol Clin

January 2025

University of Nebraska Medical Center, 42 and Emile, Omaha, NE, USA. Electronic address: https://twitter.com/pooh_velagapudi.

Women with severe aortic stenosis undergoing transcatheter aortic valve replacement (TAVR) have a higher burden of symptoms, are older at the time of the TAVR procedure, and have unique anatomic features that increase the periprocedural risk. However, long-term outcomes including mortality are favorable for women compared with men. Careful attention is needed in considering the vascular access route and choice of valve platform to mitigate complications.

Long-term motor learning in focal hand dystonia.

Clin Neurophysiol

December 2024

Human Motor Control Section, National Institute of Neurological Disorders and Stroke, National Institutes of Health, Bethesda, MD 20892, USA. Electronic address:

Objective: Because focal hand dystonia usually occurs in the over-learned stage, it would be valuable to know long-term motor learning characteristics and underlying pathophysiological features that might predispose to dystonia.

Methods: We conducted a case-control exploratory study of 15 visits over 12 weeks in the non-affected hand of a 4-finger sequence of 8 key presses in eight patients with FHD compared with eight age- and sex-matched, healthy volunteers (HVs). We studied the behavioral data and the physiological changes of the brain, including motor cortical excitability and cortical oscillations.

Development of Takotsubo cardiomyopathy following spontaneous coronary artery dissection in a patient with fibromuscular dysplasia.

BMJ Case Rep

October 2024

Internal Medicine Residency, Kaiser Permanente Mid-Atlantic States, Gaithersburg, Maryland, USA.

New-onset Takotsubo cardiomyopathy following spontaneous coronary artery dissection (SCAD) is rare. We report a middle-aged woman without significant cardiovascular risk factors, who initially presented with non-ST-elevation myocardial infarction (NSTEMI) with angiography showing sudden 'pruning' of the coronary artery consistent with SCAD. One week later, the patient returned with recurrent NSTEMI.

Background: Orthostatic hypotension is associated with cardiovascular disease. It remains unclear if low standing blood pressure or high seated blood pressure is responsible for this association. We compared associations of orthostatic hypotension and hypertension with high-sensitivity cardiac troponin I and N-terminal pro-B-type natriuretic peptide.
