Introduction: Circadian genes have an impact on multiple hormonal, metabolic, and immunological pathways and have recently been implicated in some infectious diseases.

Areas Covered: We review aspects related to the current knowledge about circadian rhythm and viral infections, their consequences, and the potential therapeutic options.

Expert Opinion: Expert opinion: In order to address a problem, it is necessary to know the topic in depth. Although in recent years there has been a growing interest in the role of circadian rhythms, many relevant questions remain to be resolved. Thus, the mechanisms linking the circadian machinery against viral infections are poorly understood. In a clear approach to personalized precision medicine, in order to treat a disease in the most appropriate phase of the circadian rhythm, and in order to achieve the optimal efficacy, it is highly recommended to carry out studies that improve the knowledge about the circadian rhythm.

One of the goals of clinical pharmacology is to optimize patient treatment by adopting new treatment strategies which will increase the efficacy of the treatment and decrease the adverse effects of the drugs. In the literature, it has shown that the effectiveness and toxicity of medications can vary significantly based on when they are administered, making timing a crucial factor in treatment plans. Chronopharmacology a relatively new branch of clinical pharmacology focuses on adjusting drug administration times to enhance patient outcomes.

Objective/goals: Cognitive decline is intricately linked to various factors such as obesity, stress, poor sleep, and circadian rhythm misalignment, which are interrelated in their impact on cognitive health. Irregular food-intake timing further compounds these issues. The practice of prolonged nightly fasting (PNF) may help synchronize food intake with circadian rhythms, potentially mitigating adverse effects of cognitive decline and associated factors.

Postoperative cognitive dysfunction (POCD) is a prevalent clinical issue following anesthesia and surgery. The onset of POCD, which is closely linked to circadian rhythm disturbance in previous studies, yet the underlying mechanism remains elusive. There is increasing evidence showed that mitochondrial architecture is coordinated by the circadian clock which DRP1 playing a crucial role.

Background: Circadian function, characterized by circadian strength, timing, and fragmentation, has been shown to correlate with cognitive function; however, little is known about in these associations, particularly among older adults who may have more variability in their sleep‐wake schedules.

Method: Among 60 community‐living adults aged 60‐90 years with self‐reported sleep complaints ≥once/ week, we investigated gender differences in the associations between circadian and cognitive function. Participants completed seven days of actigraphy and one night of polysomnography.

Background: Neuropsychiatric symptoms (NPS), including depression and circadian rhythm disruptions, are early non‐cognitive markers along the Alzheimer’s Disease (AD) continuum. These pathological states are thought to resemble AD pathogenesis, both of which are characterized by a marked decline in adult hippocampal neurogenesis.

Method: 96 elderly participants divided into three groups based on the global depression scale, neuropsychiatric inventory, clinical dementia rating, and mini‐mental status examination.
