Endometriosis is defined by the presence of functional ectopic endometrial tissue outside the uterine cavity, excluding the myometrium. It is a benign tumor that can infiltrate and cling to other organs, mimicking a malignant tumor. Umbilical endometriosis is a rare type of endometriosis that can occur naturally or as a result of a surgical operation. We report the case of a patient who experienced catamenial umbilical discomfort and whose radiological examination revealed endometriotic involvement.

A nulliparous woman in her 40s is referred to gynaecological oncology secondary to umbilical pain and bleeding with menses. Examination revealed a blood-filled cystic mass within an umbilical hernia consistent with umbilical endometrioma. The patient exhausted medical management options, then pursued surgical management via umbilectomy, excision of umbilical endometriosis, lysis of adhesions and umbilical hernia repair.

A nulliparous woman in her late 30s with a history of pericardial patch repair for atrial septal defect and completed treatment for pulmonary tuberculosis 9 years ago presented with chest pain, breathlessness and abdominal pain. Radiological imaging revealed right-sided pneumothorax, pneumopericardium and pneumoperitoneum for which an intercostal drain (ICD) was placed. A contrast-enhanced CT of the abdomen showed a distal stomach perforation, which was managed conservatively.

Primary umbilical endometriosis is a rare condition in which there is endometrial glands and stroma in the umbilicus. Primary umbilical endometriosis is also called villar's nodule.
